Heart：心血管疾病和危险因素对COVID-19患者致命性结局的影响

该荟萃分析的结果表明，对于所有年龄段的患者，CVD及其危险因素（高血压和糖尿病）与COVID-19的致命性结局密切相关。

先前评估心血管危险因素的研究认为年龄是潜在的混杂因素。近日，心脏病领域权威杂志Heart上发表了一篇研究文章，研究人员旨在根据COVID-19患者的年龄调查心血管疾病（CVD）及其危险因素对患者致命性结局的影响。

研究人员对PubMed和Embase数据库中截至2020年6月11日前发表的相关数据进行系统的文献综述和荟萃分析。除涉及儿科人群的所有观察性研究（病例研究或队列研究）外，所有研究均纳入进行分析。合并症的患病率和临床结局按每项研究的平均患者年龄进行分层（范围为：<50岁，50-60岁和≥60岁）。主要结局指标是严重COVID-19或死亡的复合致命结局。

研究人员纳入了51项研究，共48317名确诊的COVID-19患者。总体而言，患有CVD（高血压：OR为2.50，95％CI为2.15至2.90；糖尿病：2.25，95％CI为1.89至2.69）和CVD危险因素（3.11，95％CI为2.55至3.79）的患者发生严重COVID-19或死亡的相对风险显著增加。与老年患者相比，年轻患者的高血压、糖尿病和CVD患病率较低；然而，前者的致命结局相对风险较高。

由此可见，该荟萃分析的结果表明，对于所有年龄段的患者，CVD及其危险因素（高血压和糖尿病）与COVID-19的致命性结局密切相关。尽管青年患者的心血管合并症患病率低于老年患者，但高血压、糖尿病和CVD青年患者的致命结局相对危险度高于老年患者。
