肾去神经支配术治疗可明显降低高血压患者的血浆肾素活性和醛固酮水平

肾素-血管紧张素-醛固酮系统在血压(BP)调节中起关键作用,是多种降压药物的靶标。肾去神经支配(RDN)被认为中断了交感神经介导的神经激素通路,是其降低血压的部分作用机制

本研究旨在评估RDN前后的血浆肾素活性(PRA)和醛固酮,并评估这些基线神经内分泌标志物是否能预测患者对RDN的反应。

招募了确诊的未服用过降压药物治疗的患者,以比较RDN组和假手术对照组在基线和术后3个月时的醛固酮和PRA水平。根据基线PRA水平将SPYRAL HTN-OFF MED中枢试验的患者分为≥0.65 (n=110)和<0.65 ng/mL/h(n=116)两组。采用多变量线性回归模型评估RDN和假手术对照组之间的随访治疗差异。

两组患者的24h SBP和DBP的变化

RDN组与对照组的基线PRA相差无几(1.0±1.1 vs 1.1±1.1 ng/mL/h, p=0.37)。与对照组相比,RDN组患者在术后3个月时的PRA变化明显更大(-0.2±1.0 vs 0.1±0.9 ng/mL/h;p=0.001),醛固酮也有相似的变化差异(-1.2±6.4 vs 0.4±5.4 ng/dL;p=0.011)。

基线PRA对两组患者收缩压的变化的影响

在术后3个月时,RDN组与对照组相比的24小时和办公室收缩压(SBP)的治疗差异在基线PRA≥0.65 ng/mL/h的患者中大于基线<0.65 ng/mL/h的患者,尽管基线血压相近。早在RND术后2周,就可观察到根据基线PRA分组的办公室收缩压变化差异。

综上所述,与假手术对照组相比,肾去神经支配术治疗可明显降低高血压患者的血浆肾素活性和醛固酮水平。较高的基线PRA水平与办公室和24小时收缩压显著降低相关
