Circulation:射频消融后心律失常的复发率受监测方法的影响巨大!

观察到的射频消融后房性快速心律失常的复发率高度依赖于所采用的心律失常监测策略

在评估房颤 (AF) 药物和侵入性治疗干预的试验中,各种无创间歇性心律监测策略已被用于评估心律失常的复发情况。本研究(CIRCA-DOSE)旨在明确是否可确定无创心律监测的频率和持续时间,从而准确地检测心律失常的复发,并与使用植入式心脏监护仪 (ICM) 连续监测产生的房颤负担进行对比。

招募了 346 位有心律失常病史的患者,通过计算机模拟,研究者评估了一系列无创监测策略(包括在当代房颤射频消融试验中使用的监测策略)的无事件生存率、敏感性、阴性预测值和房颤负担。

各种房性心律失常的复发率

共 126,290 个监测天被纳入了本次分析。在第 12 个月时,164 位患者经历了植入式心脏监护仪(ICM)记录的房性心律失常复发(一年无事件生存率为 52.6%)。房颤射频消融试验中使用的大部分监测策略监测心律失常复发的敏感性较差。敏感性随着监测强度的增加而增加,短持续时间监测器(24/48小时 ECG 监测)会漏掉很大一部分复发(敏感性分别是 15.8% [95% CI, 8.9%–20.7%] 和 24.5% [95% CI, 16.2%–30.6%])。

不同监测策略的敏感性

较长持续时间监测器(14天 ECG 监测)的心律失常复发的捕捉率接近于植入式心脏监护仪(敏感性 64.6% [95% CI, 53.6%–74.3%])。来自短持续时间监测器的房颤负担显著高估了复发患者的真实房颤负担。

总之,观察到的射频消融后房性快速心律失常的复发率高度依赖于所采用的心律失常监测策略。连续长期(7-14天)间断性的监测(一年至少累计28天)可获得与植入式心脏监护仪相当的房颤负担估计。但是,植入式心脏监护仪在检测心律失常方面还是优于间歇性监测,应作为临床试验的金标准。
