Lancet子刊:意外——“万金油”维生素D并不能降低死亡率!

综上,每月给未经筛查的老年人服用维生素D3并不能降低全因死亡率。

随着生活水平的提高,人们对于营养状况的平衡越来越重视。有研究显示,高达52%的人在服用各种营养补剂。其中,31%的人选择多种维生素,包括维生素D、钙和维生素C等。欧洲癌症与营养前瞻性调查协会曾在2009年对36034名男性和女性的数据进行调查,发现补充剂的使用范围很广,不仅有地域差别,而且存在明显的性别差异,女性使用补充剂的比例往往更高。

目前,有关给未经筛查的成年人补充维生素D3对死亡率的影响尚不清楚。为了确定每月剂量的维生素D3是否会影响老年人的死亡率,来自澳大利亚的专家开展了一个随机的、双盲的、安慰剂对照的口服维生素D3补充试验(每月60000 IU),结果发表在Lancet Diabetes & Endocrinology杂志上。

参与者被随机分配(1:1),每月接受一次60000 IU维生素D3的口服凝胶胶囊或安慰剂,为期5年。主要终点是评估所有被随机分配的参与者的全因死亡率。此外还分析了癌症、心血管疾病和其他原因的死亡率。

在2014年2月14日至2015年6月17日期间,随机分配了21315名参与者,包括10662人到维生素D组,10653人到安慰剂组。在随访期间从随机抽样的参与者(N=3943)收集的4441份血样中,安慰剂组的平均血清25-羟基-维生素D浓度为77,维生素D组为115nmol/L。

干预5年后(中位随访5.7年[IQR 5.4-6.7]),记录了1100例死亡(安慰剂538[5.1%];维生素D 562[5.3%])。总的来说,维生素D3对全因死亡率没有影响,(HR=1.04,95% CI 0.93-1.18];p=0.47),同样对心血管疾病死亡率也没有影响(HR=0.96, 95% CI 0.72-1.28;p=0.77)。进一步分析在癌症、其他原因的死亡率上均没有差异(HR=1.15, 95% CI 0.96-1.39; HR=0.83,95% CI 0.65-1.07)。

随访期间,维生素D与对照组的死亡率变化

每个方案分析的几率是OR =1.18(95% CI 1.00-1.40;p=0.06)。在排除前两年随访的探索性分析中,那些被随机分配到接受维生素D的人,其癌症死亡的危险性甚至比安慰剂组的人要高(HR 1.24 , 95% CI 1.01-1.54)。

综上,每月给未经筛查的老年人服用维生素D3并不能降低全因死亡率。
