JAHA:生命早期危险因素与房颤的相关性研究

早期生命危险因素与心脏代谢疾病有关,但在房颤(AF)方面尚未得到充分研究。 现有的出生体重和房颤的研究结果不一致,母体体重、胎龄、胎盘大小和出生长度的影响是未知的。本研究旨在评估分析早期的生命危险因素与房颤之间的相关性。

早期生命危险因素与心脏代谢疾病有关,但在房颤AF)方面尚未得到充分研究。 现有的出生体重和房颤的研究结果不一致,母体体重、胎龄、胎盘大小和出生长度的影响是未知的。本研究旨在评估分析早期的生命危险因素与房颤之间的相关性。

赫尔辛基出生队列研究纳入了1934-1944年出生的13345名研究对象。后续的研究是通过国家登记册,并于20131231日结束,共有907起事件。并根据围产期变量和事件性房颤构建出生年份分层的Cox回归分析,根据子女的性别、孕龄和出生时的社会经济地位进行调整分析。研究结果发现出生体重与房颤呈U型相关性(p=0.01),与房颤相关性最低的出生体重是3.4kg(女性为3.8kg,男性为3.0kg)。孕期体重指数高(30 kg/m2)的孕妇的子代患房颤的发病率更高(HR1.3695% CI 1.07–1.74, P=0.01)。孕妇身高与早发性房颤(<65.3岁)相关,危险比为1.4795CI 1.24-1.74P <0.0001),但与晚发的AF无关。本研究结果结果独立于冠状动脉疾病、高血压糖尿病

孕期母亲的高体重指数和身高是新报道的预测后代AF的指标。 预防母亲的肥胖可能会降低子孙后代AF的发生。 出生体重与其他围产期变量事件与AFU形相关。
