NEJM:瑞德西韦可缩短新冠肺炎患者恢复时间

存在下呼吸道感染证据的新冠肺炎患者,瑞德西韦治疗在缩短康复时间方面有益

目前尚无有效的2019年冠状病毒病(Covid-19)药物治疗手段,曾经瑞德西韦是最被业界给予厚望的药物。近日研究人员公布了瑞德西韦治疗新冠肺炎临床研究的最终结果。

在这项双盲、随机、安慰剂对照研究中,研究人员招募了因Covid-19住院并伴有下呼吸道感染的成年患者,随机静脉注射瑞德西韦(第1天负荷剂量200毫克,随后每天维持治疗剂量为100毫克,最多9天)或安慰剂。研究的主要结果是恢复时间,定义为出院或仅用于控制感染目的的住院。

总共有1062名患者参与研究,其中瑞德西韦组541人,安慰剂组521人。 接受瑞德西韦治疗患者的中位恢复时间为10天,而接受安慰剂患者的中位恢复时间为15天 (恢复率为1.29)。分析显示,与安慰剂相比,接受瑞德西韦治疗患者在第15天出现临床改善(实际疾病严重程度调整后优势比1.5)的可能性更高。生存分析显示,瑞德西韦组15天死亡率为6.7%,安慰剂组为11.9%,瑞德西韦组第29天死亡率为11.4%,安慰剂为15.2%(危险比为0.73)。在532名接受瑞德西韦治疗的患者中,有131名(24.6%)报告了严重不良事件,516名接受安慰剂治疗的患者中有163名(31.6%)报告了严重不良事件。

研究认为,存在下呼吸道感染证据的新冠肺炎患者,瑞德西韦治疗在缩短康复时间方面有益。
