J Endod:CBCT对颈部外吸收治疗方案以及一项新型轴向片层分类系统的影响

这篇研究的目的是为了比较根尖X线片(PA)与CBCT影像对颈部外吸收(ECR)牙齿治疗计划的影响。第二个目的是为了测试一项全新的基于CBCT技术针对ECR的轴向片层分类系统。研究从数据库中鉴定出56颗ECR牙齿(47名患者)共928份CBCT影像。在执行严格的淘汰标准后,筛选出30颗ECR牙齿(25患者)和10颗未罹患ECR的对照牙齿。由6名检测员对CBCT影像及相匹配的PA影像独立进行评估。检测者按照Heithersay和全新的Rohde分级系统对ECR进行分级和制定治疗方案。结果显示,CBCT鉴定出全部30颗ECR牙齿,PA X线摄像术鉴定出29颗。测试者间使用CBCT的鉴定结果高度一致,并且通过CBCT扫描所制定的治疗计划有56.7%不同于PA X线片。检测者建议了大多数ECR病例的修复(59.8% CBCT影像和56.7% PA X线片)。依赖于CBCT影像对ECR牙齿进行的Heithersay等级分类更普遍的定义为4级(P = .0016)。并且Rohde分析系统可以明显预测到所建议的治疗计划(P = .002 for Rohde class 2和P = .043 for R
