Radiology:孕妇高浓度吸氧会对先天性心脏病胎儿胎盘、颅脑供氧有作用吗?

先天性心脏病(CHD)胎儿会在子宫里因脑供氧不足而出现脑发育受损。

背景

先天性心脏病(CHD)胎儿会在子宫里因脑供氧不足而出现脑发育受损。

目的

本研究旨在评价通过孕期高氧治疗能够提高胎儿颅脑供氧量,利用血氧水平依赖(BOLD)功能MRI来评价孕期高氧摄入对胎盘和胎儿颅脑的影响。

 

材料与方法

对86例胎儿(56例健康胎儿和30例诊断CHD的胎儿)BOLD MRI检查。试验设计分为:I期,2分钟平静状态(室内空气);II期,孕妇摄入100%氧气6分钟;III期,再次平静状态下呼吸5-6分钟。经过运动矫正后,对胎盘和胎儿颅脑信号取平均值,转换为R2*信号变化(ΔR2*)。CHD胎儿被分为单心室(SV)和双心室(TVs)以及主动脉狭窄(AO)和非AO的患者。利用广义线性混合模型进行数据分析。  

结果

在孕妇输入纯氧后健康胎儿和CHD胎儿的胎盘ΔR2*均上升,但在SV CHD胎儿升高更明显些(健康胎儿、SV CHD胎儿和TV CHD胎儿平均ΔR2*分别为、1.3 sec-1 ± 0.1 [standard error; P < .01]、1.9 sec-1 ± 0.2 [P < .01]和1.0 sec-1 ± 0.3 [P < .01)。在健康胎儿和SV CHD胎儿或AO CHD胎儿中胎盘平均 ΔR2*随着GA变化而变化,但TV CHD或非AO CHD患者中并无相应的变化。健康胎儿、和TV CHD胎儿颅脑 ΔR2*在各期保持一致,而在SV 或AO CHD胎儿在孕妇高氧摄入后ΔR2*升高(平均ΔR2*分别为0.7 sec-1 ± 0.2 [P = .01] 、0.5 sec-1 ± 0.2 [P = .02])。

结论

经过孕妇高氧摄入后健康胎儿和先天性心脏病的胎儿胎盘氧含量均增加,而对于单心室或主动脉狭窄的CHD患者出现了选择性脑血氧含量增加。
