Burns:自体脂肪移植在急性烧伤伤口处理中的效果

脂肪移植正被广泛用于不同的疾病治疗,其中之一就是促进伤口愈合。一些小型的观察性研究表明,自体脂肪移植对慢性伤口的愈合、不同的血管溃疡或对疤痕的影响,但还没有随机对照试验来比较其与传统治疗方法的效果差异。近日,发表于Burns的一项研究中,研究人员将自体脂肪移植(AFG)作为急性烧伤伤口愈合和烧伤疤痕的新适应症,与传统的烧伤伤口处理方法比较临床和组织学上的治疗差异。

该研究是一项前瞻性、开放标签的单中心、随机对照临床试验,纳入2019年3月至2020年3月浅层和深层真皮烧伤的100名患者,随机接受AFG方案,包括单次注射自体脂肪移植,然后用纳米脂肪包扎(A组)或传统治疗方法——1%磺胺嘧啶银或其他外用药连续包扎(B组)。纳入标准包括新入院的烧伤患者,烧伤总表面积(TBSA)(10%-25%),而排除标准包括烧伤TBSA<10%或>25%的烧伤患者,或皮下脂肪、筋膜、肌肉和骨骼丧失,吸入性烧伤,生殖器、会阴和肛门周围的烧伤,以及存在可能影响伤口愈合或麻醉和手术的合并疾病(ies)。同时,通过光镜检查对两组样本进行组织学分析,确认结果,并对纳米脂肪进行流式细胞分析,分析间质干细胞标记物CD90、CD44、CD45、CD73和CD34的分化簇(CD)标记。

 

结果显示,与B组患者相比,A组患者的总住院天数明显减少(p = <0.001),进一步植皮的次数减少(p = 0.003),挛缩形成减少(p = <0.002),而疤痕纹理得到改善(p = <0.001)。流式细胞分析表明,纳米脂肪细胞的CD 90、CD 73、CD 44、CD 45和CD 34标志物呈阳性。

综上所述,该研究结果表明,在AFG方案与传统方法治疗急性烧伤伤口的比较中,AFG方案与明显的临床改善有关,表现为较少的住院时间、较低的疤痕或挛缩发生率和较少的皮肤移植使用量。
