JCEM:补充维生素D对糖尿病发病率的影响

在糖尿病前期患者中,与安慰剂相比,中等至高剂量(> 1000IU/d)而不是低剂量维生素D补充可显著降低T2DM的发病风险。

补充维生素D对2型糖尿病(T2DM)风险的影响仍然存在争议,因为大多数随机对照试验(RCT)较小型或者只报道了低剂量维生素D的效应。近日,内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章,研究人员进行了RCT的荟萃分析,并评估维生素D补充剂在预防T2DM中的作用。

两名研究人员对Pubmed、MEDLINE、EMBASE和Cochrane图书馆等数据库进行了检索,检索时间从数据库成立到2019年9月15日。研究人员纳入的RCT报告了补充维生素D至少一年对预防T2DM的作用。两名独立的研究者提取了数据,并报告了风险比(RRs)和95%置信区间(CIs)。荟萃分析的主要结局是T2DM的发生率。

该分析共纳入了九个RCT(43559名参与者)。平均年龄(SD)为63.5(6.7)岁。与安慰剂相比,维生素D的RR为0.96(95%CI为0.90-1.03); P=0.30。在中等至高剂量(≥1000IU/d)的试验中,所有试验均在糖尿病前期受试者中进行,维生素D与安慰剂相比的RR为0.88(95%CI为0.79-0.99)。反之,在一般人群样本中进行的低剂量试验并未发现风险降低(RR为1.02 [95%CI为0.94-1.10]);剂量相互作用P=0.04。
