Biomaterials:沈华浩团队发现纳米药物有望成为治疗哮喘的潜在靶向新药

近日,浙江大学医学院附属第二医院呼吸与危重症医学科沈华浩、应颂敏教授团队与浙江大学药学院凌代舜教授团队通过多学科交叉研究发现:Bcl-2抑制剂小分子的纳米型药物能够很好的治疗哮喘的气道炎症和气道高反应性,相关论文Nanoformulated ABT-199 to effectively target Bcl-2 at mitochondrial membrane alleviates airway inflammation by inducing apoptosis于国际顶级材料学期刊《生物材料》发表(Biomaterials 2019,192:429-439,5年影响因子:9.315)。这一发现将为研发新型的、特异的潜在哮喘靶向药物带来希望。这是继沈华浩、应颂敏教授团队于之前发表于变态反应学领域全球Top 1专业期刊《变态反应与临床免疫学杂志》(Journal of Allergy and Clinical Immunology 2017,5年影响因子:12.704),关于重症哮喘研究工作的延续。

曾作为中华医学会呼吸病学分会哮喘学组组长和牵头人领衔完成《中国哮喘防治指南》制定的沈华浩教授介绍,全世界大约有3亿哮喘患者,中国约有3千万,我国是全球哮喘病死率最高的国家之一。目前仍有约占整个哮喘群体的5%~10%的患者对常规治疗的反应不佳,其所花费的医疗资源占据50%以上,是导致哮喘治疗费用增加的主要原因之一。目前重症哮喘患者控制水平差,严重影响生活质量,占用巨额医疗资源,加重社会经济负担,是哮喘致残、致死的主要原因。为这一部分患者寻找对症方案是需要紧迫解决的临床需求。

本研究通讯作者之一应颂敏教授介绍,哮喘发病时会引起气道平滑肌的痉挛,气道内气流不通畅,导致喘息、气促、胸闷和(或)咳嗽等症状。课题组研究发现,哮喘气道周围聚集了大量的炎症细胞,而且这些细胞的存活能力明显增强。简单的讲,有效治疗哮喘的一个策略就是要靶向诱导这类炎症细胞的凋亡或死亡,把炎症细胞的数量降下来。

在该研究中,课题组利用高分子聚合物作为纳米药物递送系统将Bcl-2抑制剂ABT-199有效包载,最终得到了pH敏感型线粒体靶向的纳米药物。课题组研究发现,ABT-199纳米型药物通过气道给药方式给予哮喘动物模型,这些药物会被炎症细胞摄取,从溶酶体中进一步逃逸,最后通过静电作用靶向于线粒体,增强对Bcl-2蛋白的抑制,促进炎症细胞凋亡,进而有效抑制气道炎症与气道高反应性。沈华浩教授强调,尽管相关研究仍处于动物实验阶段,但通过进一步临床试验,这一小分子将来有望成为治疗人类重症哮喘的一种靶向新药;同时也为纳米药物应用于气道炎症疾病的治疗提供了新的思路。
