Ann Neurol:视网膜厚度可预测帕金森氏病认知下降的风险

近日,神经病学领域权威取杂志Annals of Neurology上发表了一篇研究文章,研究人员旨在分析视网膜厚度的纵向变化及其作为特发性帕金森病(iPD)疾病进展生物标志物的预测价值。

该研究招募了路易体病(LBDs)患者并对其进行了3年的前瞻性评估,包括iPD(n=42),路易体痴呆(DLB,n=4),E46K-SNCA突变携带者(n=4)和对照者(n=17)。所有参与者均进行了光谱视网膜光学相干断层扫描和蒙特利尔认知评估(MoCA),并获得了患者的帕金森氏病统一疾病评估量表(UPDRS)评分。研究人员采用线性混合模型估计了黄斑神经节内丛状复合物(GCIPL)和乳头周围视网膜神经纤维层(pRNFL)的厚度减少率,并使用临床意义上的临界值,计算风险比以评估基线GCIPL和pRNFL厚度与随后认知和运动恶化风险之间的关联。

中央凹旁区域(1-3mm环)的GCIPL厚度萎缩程度最大。iPD患者的年平均萎缩率为0.63μm,对照组为0.23μm(p<0.0001)。基线时中心凹下GCIPL和pRNFL厚度较低的iPD患者在3年后出现认知下降的风险增加(RR为3.49,95%CI为1.10-11.1,p=0.03;RR为3.28,95%CI为1.03-10.45,p=0.045)。研究人员没有发现视网膜厚度与运动能力恶化之间存在显著关联。

由此可见,该研究的结果提供了使用OCT测量中心凹旁GCIPL厚度来监测神经变性并预测iPD随时间推移认知恶化风险的证据。
