NJCP:脐血白蛋白可作为新生儿黄疸的预测因子

根据目前的研究结果,血清脐血白蛋白是高胆红素血症和早期黄疸评估的一个很好的预测因子,特别是在难以定期随访的不发达国家。

脐血白蛋白作为新生儿黄疸严重程度预测指标的研究很少。当间接胆红素水平(UCB)超过20 mg/dl时,可能会发生神经毒性,但在较低水平时也可能发生。UCB与白蛋白紧密结合,有助于其在血浆中的保存。当UCB与白蛋白的比例增加时,进入细胞的非白蛋白结合或游离UCB的数量增加并引起毒性。Nigerian journal of clinical practice杂志发表了一项研究,旨在探讨脐血白蛋白是否可用于预测需要光疗或换血的新生儿高胆红素血症的发生。

在2020年1月1日-11月1日期间,在纳杰夫市的教学医院进行的一项队列研究。随机入组100例足月健康新生儿。随访时间在出生后5天内,排除标准包括新生儿败血症、出生窒息、器械分娩(产钳或真空)、呼吸窘迫、羊水胎粪污染、出生后24小时内新生儿黄疸、Rh和ABO血型不合以及黄疸家族史。收集患儿姓名、性别、胎龄、体重、Apgar评分、生后第1天脐血白蛋白、出生后第3、5天血清总胆红素(TSB)。

研究纳入的100例健康足月新生儿,60例脐血白蛋白低(<2.8 g/dl), 40例脐血白蛋白正常(≥2.8 g/dl)。脐血白蛋白低的新生儿中出现严重黄疸需要治疗的比例为66.6%,而脐血白蛋白正常的新生儿中有12%需要治疗。脐血白蛋白低与新生儿第3天发生显著黄疸有显著相关性,脐血白蛋白低的新生儿发生显著黄疸的风险是脐血白蛋白正常新生儿的5倍。

根据目前的研究结果,血清脐血白蛋白是高胆红素血症和早期黄疸评估的一个很好的预测因子,特别是在难以定期随访的不发达国家。
