既往研究显示,2型糖尿病的患病率具有明显的性别差异,绝经前女性的发病率明显较低。此外,雌激素缺乏可能与代谢功能障碍之间存在较强的相关性。绝经后女性雌激素的减少可加速胰岛素抵抗及2型糖尿病的发生;对绝经后女性行雌激素替代治疗则有助于改善胰岛素抵抗,降低血浆葡萄糖水平。那么,对于女性而言至关重要的雌激素到底在2型糖尿病的防治中又能发挥怎样的角色?其作用机制是什么?德克萨斯A&M 大学的Shaodong Guo等人在 Diabetes 上发表的一项最新研究为我们提供了答案。

该研究通过对雄性及卵巢切除的雌性对照小鼠及肝脏特异性敲除转录因子Foxo1的小鼠进行研究发现,雌激素可通过转录因子Foxo1减轻胰岛素抵抗和减少糖异生而非通过促进肌肉葡萄糖摄取来减少2型糖尿病的发病;Foxo1是雌激素有效抑制糖异生所必需的。这表明,雌激素是通过激活雌激素受体信号来抑制肝脏葡萄糖产生的,上述作用可能与胰岛素受体底物Irs1和Irs2无关。

该研究结果对于人们深入认识肥胖/糖尿病及其有潜力的饮食干预具有深远意义,提示以雌激素为靶点的治疗措施(如靶向雌激素受体ERα)有望成为调节糖代谢和预防糖尿病的新途径。目前来说,雌激素相关治疗的副作用(可能增加乳腺癌卒中风险)是其用于糖尿病防治所面临的主要障碍。了解雌激素的组织特异性及其参与代谢调节的分子机制(确定雌激素受体的直接靶标)则有助于研发有针对性的雌激素模拟物和新型选择性配体,从而在有效防治糖尿病的同时,避免不必要的副作用的发生。
