Diabetes Care:糖尿病夏柯氏足患者骨折和骨质疏松症风险

与没有夏柯氏足的糖尿病患者相比,糖尿病夏柯氏足患者发生骨折和骨质疏松症的风险增加。

夏柯氏足是糖尿病的严重并发症,会导致足部和脚踝部位的骨骼和关节退化。尚不清楚糖尿病夏柯氏足患者发生骨质疏松症和骨折的风险是否会增加。

近日,糖尿病领域权威杂志Diabetes Care上发表了一篇研究文章,该研究旨在调查与没有夏柯氏足的糖尿病患者相比,糖尿病夏柯氏足患者骨折和/或骨质疏松症的风险是否会增加。


研究人员进行了一项基于丹麦注册的全国性人群匹配队列研究。在1995-2018年期间,研究人员确定了1602名糖尿病夏柯氏足患者,并根据性别和糖尿病诊断日期将他们与16296名没有夏柯氏足的糖尿病患者进行了匹配。研究人员使用Logistic回归来估计骨折和骨质疏松症的比值比(OR)和95%CI。研究人员从丹麦国家患者登记处检索了有关暴露、结局和合并症的信息。

糖尿病夏柯氏足患者的骨折风险
 

与没有夏柯氏足的糖尿病患者相比,糖尿病夏柯氏足患者发生骨折的风险更高(即,任何骨折、小腿骨折、足部骨折和骨质疏松性骨折的OR分别为1.8[95%CI1.6-2.0]、2.4[2.0-2.8]、2.9[2.6-3.3]和1.3[1.1-1.4])。此外,与没有夏柯氏足的患者相比,糖尿病夏柯氏足患者患骨质疏松症的风险更高,OR为1.3(95%CI为1.1-1.5)。
 
由此可见,与没有夏柯氏足的糖尿病患者相比,糖尿病夏柯氏足患者发生骨折和骨质疏松症的风险增加。
