Radiology:放射组学分析在钆塞酸增强MRI肝硬化中分级中的价值

本研究旨在建立和验证利用钆塞酸增强肝胆期MRI基于放射组学模型对肝硬化分级的价值。

本研究共回顾性分析了436例经病理分析证实为肝硬化并行钆塞酸增强MRI的患者,并以3:1随机分为建立组(n = 329)和测试组(n = 107)。在建立组,利用Logistic回归和弹性网络回归模型建立计算放射组学肝硬化指数(RFI)的模型。弹性网络回归模型用来区分F3-F4与F0-F2。利用ROC曲线分析确定诊断临床肝硬化(F2-F4)、肝硬化晚期(F3-F4)和肝硬化(F4)的最佳截断值。在测试组,利用Obuchowski指数比较RFI与标准肝增强、天门冬氨酸比血小板指数(APRI)和肝硬化-4指数。

结果为,在测试组,RFI (Obuchowski 指数为 0.86)要明显优于标准肝增强(Obuchowski 指数为 0.77; P < .03)、APRI(Obuchowski 指数为 0.60; P < .001)和肝硬化-4指数(Obuchowski 指数为 0.62; P < .001)。通过利用截断值,RFI在诊断F2-F4的敏感性和特异性分别为81% (95% CI为71%, 89%) 、 78% (95% CI为63%, 89%),在诊断F3-F4的敏感性和特异性分别为79% (95% CI为67%, 88%) 、 82% (95% CI为69%, 91%)、在诊断F4的敏感性和特异性分别为92% (95% CI为79%, 98%) 、 75% (95% CI为62%, 83%)。

本研究表明,钆塞酸MRI肝胆期放射组学分析能够准确诊断肝硬化。
