肥胖是2型糖尿病的一个危险因素，但对于肥胖的时间和累积暴露与疾病风险的关系仍知之甚少。本研究的目的是研究成年早期的BMI轨迹、肥胖发病年龄和肥胖年龄(肥胖程度和肥胖持续时间的乘积)与随后的2型糖尿病风险之间的关系。

每3年通过长达19年的调查，对1996年参与澳大利亚妇女健康纵向研究(ALSWH)的18-23岁基线妇女(n = 11,192)进行随访。收集了七次自我报告的体重。由患者自行报告2型糖尿病的发病。使用生长混合模型来确定成人早期生命过程中不同的BMI轨迹。使用Cox比例危害回归模型来研究轨迹和糖尿病风险之间的关联。

在平均16年的随访中，162名(1.5%)妇女被新诊断为2型糖尿病。确定了6种不同的BMI轨迹，不同的初始BMI和不同的增长斜率有所不同。初始BMI与糖尿病风险呈正相关。我们还观察到，开始肥胖的年龄与糖尿病风险呈负相关(HR 0.87 [95% CI 0.79, 0.96]每1年增加)，肥胖年龄与糖尿病风险呈正相关(p趋势<0.0001)。

研究数据揭示肥胖的时机和肥胖的累积暴露对年轻女性2型糖尿病发展的重要性，这表明预防或延迟肥胖的发作并减少肥胖的累积暴露可能会大大降低患上糖尿病的风险。
