J HEPATOLOGY:利福昔明可以减少肝硬化和肝性脑病患者的肠道源性炎症和粘蛋白降解程度

      肝硬化晚期会带来很多并发症,包括肝性脑病 (HE)、静脉曲张出血、腹水等,这会导致多器官功能衰竭的出现。肠道微生物组在肝硬化的发病机制中起着至关重要的作用,肝硬化患者会从健康的肠道微生物组演变为以肠道微生物活动失调或与肝硬化失代偿相关的“生态失调”为特征的肠道微生物组。细菌易位(BT)是肝硬化相关免疫功能障碍(CAID)的重要驱动因素。不可吸收的抗生素利福昔明-α显著降低了HE 复发的风险和住院的几率,但其作用机制尚不清楚。因此,本项研究旨在对此进行深入探究。

 

      研究人员在肝硬化并伴有HE 患者中进行了一项利福昔明与安慰剂治疗的随机对照双盲机制研究。治疗组给予利福昔明-α 550mg (TARGAXAN) 每天两次 (n=19) ,对照组则给予安慰剂 (n=19) 给药 90 天。主要观察结果:30 天时中性粒细胞氧化爆发 (OB) 减少 50%的几率。次要观察结果:心理测量肝性脑病量表 (PHES)、唾液和粪便的鸟枪宏基因组测序、血浆和粪便代谢组、血液细菌 DNA、中性粒细胞 Toll 样受体 (TLR)-2/4/9 和白细胞介素-8 表达以及血浆和粪便细胞因子分析的差异。

 

      研究结果显示,治疗第 30 天肝硬化患者的 HE等级在利福昔明-α治疗组中正常化,但安慰剂组中变化不明显(p = 0.014),而且福昔明-α治疗组的患者其PHES 评分有所改善。利福昔明-α降低了患者循环中性粒细胞 TLR-4 表达 (p = 0.021),同时降低血浆肿瘤坏死因子-α (TNF-α);利福昔明-α抑制了肠道的炎症,主要表现在降低了粘蛋白降解富含唾液酸酶物种链球菌属的浓度. 同时利福昔明-α 促进富含 TNF-α 和 IL-17E 的肠道微环境,增强对入侵病原体的抗菌反应并促进肠道屏障修复。研究结果也发现服用利福昔明-α 的患者发生感染的可能性较小 [OR,0.21(0.05-0.96)]。

 

      本项研究证实利福昔明治疗的患者发生感染的可能性较小,显性和隐性 HE 消退的程度更为明显。利福昔明-α 减少了黏蛋白降解物质减轻了全身炎症。这些数据都表明利福昔明-α通过修复肠道屏障改善了肝硬化中的细菌易位和全身性内毒素血症。
