Obes Rev:胃绕道术后体积骨矿物质密度和骨质的变化

肥胖是多种健康的危险因素,如2型糖尿病、非酒精性脂肪肝、冠状动脉疾病、阻塞性睡眠呼吸暂停和骨关节炎。减肥手术(BS)是治疗严重肥胖症的最有效方法,可以改善生活质量、死亡风险和相关的并发症。近日,发表于Obes Rev的一项系统回顾和元回归分析旨在确定Roux-en-Y胃绕道术(RYGB)对三维评估的体积骨矿物质密度(vBMD)的影响,以及时间对这些变化的影响,对骨质的影响,以及双能X射线吸收仪(DXA)与定量计算机断层扫描(QCT)或高分辨率外周QCT(HR-pQCT)对骨量损失估计的一致性。

研究人员系统搜索了PubMed、Web of Science、Cochrane、Scopus和EBSCO中收录的相关文章。纳入了对接受RYGB治疗的成人的纵向研究,这些研究通过QCT或HR-pQCT评估vBMD,并随访≥6个月。

结果,共有四项研究报告了全髋关节(TH)的变化,八项报告了腰椎(LS)的变化,八项报告了桡骨的变化,七项报告了胫骨的变化。在分析的所有骨骼部位,RYGB后的vBMD都有明显的减少。荟萃回归显示,RYGB后的时间与所有骨骼部位的vBMD恶化明显相关,除了TH部位。RYGB也导致了骨质量的明显恶化。DXA低估了RYGB后的LS,高估了TH的骨质损失。

综上所述,该研究结果表明,RYGB与明显的vBMD损失有关,这使得通过三维技术筛查骨量进展成为预防骨折风险和骨质疏松症的关键临床问题。
