中药糖尿病创新药——桑枝总生物碱,来龙去脉,你知道吗?

会议邀请了北京协和医院教授梁晓春、北京大学人民医院教授纪立农分别就“桑枝总生物碱片临床研究”和“天然药物治疗糖尿病的优势与展望”作了精彩的学术报告。

今年3月,国家药品监督管理局批准了桑枝总生物碱片的上市注册申请。该药的主要成份为桑枝中提取得到的总生物碱,配合饮食控制及运动,用于2型糖尿病。北京五和博澳药业有限公司为该品种的药品上市许可持有人。该药主要成份为桑枝中提取得到的总生物碱,临床试验结果显示与安慰剂对照组间比较有统计学差异,可有效降低2型糖尿病受试者糖化血红蛋白(HbA1c)水平。

桑枝生物碱是以种桑养蚕桑树管理中的废弃物“桑枝”为药材,经提取分离纯化而得,属于天然水溶性生物碱,对a-葡萄糖苷酶具有极强的抑制活性且靶点更为精准,用于2型糖尿病治疗,物质基础明确,质量可控,靶点清楚。有糖苷酶抑制作用,能调节脂代谢,保护胰岛细胞,促进GLP-1分泌,改善胰岛素抵抗,改善肠道微生态。

阿卡波糖(Acarbose)是一种最常用的α-葡萄糖苷酶抑制剂,桑枝生物碱作用机制与其相似。尤其是亚洲人以碳水化合物为主的饮食结构而言,具有较好的降糖作用。

桑枝生物碱通过与小肠刷状缘的糖苷酶可逆性竞争结合,抑制食物中的寡糖及双糖降解为萄萄糖,减少血糖升高与波动。

II、III期临床试验结果显示,该药疗效确切,有良好的剂量效应关系;不会引起低血糖,且较少吸收入血,长期用药的肝肾毒性风险较低;与化药相比,对糖苷酶选择性更强,胃肠胀气不良反应明显降低。长期服用本品不仅可显著降低空腹及餐后血糖,还可有效控制“糖化血红蛋白”,减少或延缓并发症的发生与发展。

桑枝总生物碱片于2008年9月获批开展临床试验,直至2017年向国家药监局药品审评中心(CDE)正式提交上市申请,经历了长达十年的临床试验。II、III期临床试验,均由北京协和医院牵头,31家临床机构共同参与完成。研发团队选择了国际公认的降糖金标准“糖化血红蛋白”为主要疗效指标,以主流口服降糖药物阿卡波糖作为阳性对照药,开展了头对头的随机双盲临床评价。

“严格的临床试验表明,桑枝生物碱对α-葡萄糖苷酶具有极强的抑制活性。无论单独使用还是用于二甲双胍控制不佳的联合治疗,该产品均显示了良好的降糖化血红蛋白疗效,降低值达0.93%,疗效堪与化学药阿卡波糖相媲美。值得关注的是,桑枝总生物碱的靶点选择性强,可精准作用于小肠双糖酶而不抑制α-淀粉酶的活性,因此能显著降低服药之后胃肠胀气、排气等令人尴尬的胃肠不良反应发生率。”刘玉玲表示。此外,按照国际上对创新药临床评价的要求,研发团队还开展了与安慰剂比效的随机双盲临床试验,验证去除安慰剂效应后的绝对疗效。

桑树不同的部位均可入药。早在《本草纲目》中,就有关于桑白皮和桑叶用于消渴症的记载。上世纪90年代初,申竹芳与药物所专家谢明智教授率先在国内建立了糖苷酶抑制剂筛选方法。通过对百余种常用治疗糖尿病的中药进行药理活性筛选,首次发现了桑枝具有很强的糖苷酶抑制活性,这一研究结果为桑科植物可用于治疗消渴病提供了明确的现代医学理论依据,也为以桑枝为药材开发降糖药物奠定了基础,“

直到1999年,“桑枝总生物碱及其制剂”研究正式立项,由刘玉玲与申竹芳领衔,展开深入系统的研究,研发物质基础明确、质量可控、具有临床优势的中药创新药。

研发团队遇到的第一个难题,就是物质组成及其质量控制。桑枝生物碱为水溶性的多羟基生物碱,没有紫外吸收。再加上桑枝中含有丰富的氨基酸,结构与多羟基生物有些类似,对质量控制造成困扰。除生物碱外,桑枝中还含有多种多种水溶性成分,组成极为复杂。生物碱的化学结构、含量及其组成是什么?药物所经多学科协作,最终建立了科学的仪器分析方法,准确鉴定了生物碱的化学结构,对生物碱的组成比例及含量进行了准确控制,还对非生物碱组分进行定性和定量。最终,生物碱和非生物碱组分的定量之和,达到提取物的90%以上。

研发团队遇到的第二个难题,就是提取纯化工艺及中试放大的难题。无论是桑叶、桑根皮还是桑枝,生物碱的含量都非常低。微量水溶性活性成分的纯化富集,是桑枝总生物碱项目发展的难点之一。研发团队开展了产学研合作,联合攻关,攻克了成果转化和系统集成的高技术壁垒,从药材进厂、粉碎、提取,到分离、纯化、精制和干燥,实现了全过程自动化和精细化的过程控制,提取率和产品质量稳定,生产规模可满足年产5亿片的要求。

桑枝总生物碱项目的成功研发,不仅为重大专项提供了标志性成果,还实现了桑枝的变废为宝,算是中药深开发的曲范性项目。

梅斯医学小编点评:

创新药不容易,发现一款创新药,需要漫长时间和持续研发。就目前而言,桑枝总生物碱应该是走完第一步,因为它仅仅与阿卡波糖相似(降糖效果,大剂量情况下类似阿卡波糖,不良反应略轻微一些),现在降糖药物都已地板价了,桑枝总生物碱限于成本(提取的成本应该不低),未来的销量尚难以预测。但是,这条路子探索是对的,目前是桑枝总生物碱,应该在此基础上,进一步筛选单个活性最高的小分子,并进一步进行化学改造,转化为第二代“桑枝XX碱”,以使其更有竞争力。正如今天大家看到的二甲双胍,也是从植物中通过N代改良而来,从十胍,到苯乙双胍,然后才是二甲双胍(详细见:二甲双胍:过去、现在与将来)。包括现在最常用的SGLT2i最初也是源自植物中的“根皮苷”,一步步改造而成。详细见:由根皮苷到坎格列净:来源于植物的神奇降糖新药

如果桑枝总生物碱就此止步,那是极为可惜的。期待不断研发深入,成就新一代降糖药物。

当然,从品牌角度上看,逐步可以脱离“桑枝”的阴影。如果是纯的小分子,完全可以是化学药物了。民族特色,不一定是最好的。事实上二甲双胍,SGLT2i类药物最初来自于植物,已经没有多少人知晓了,并不妨碍它成为糖尿病中的经典药物。
