JAMA子刊:炎症可能通过激活小胶质细胞和/或星形细胞功能障碍导致大脑结构的变化

由基因决定的IL-6与大脑结构有关,并可能影响发育期神经精神疾病(包括精神分裂症和自闭症)所涉及的区域。

既往研究表明,在一系列神经精神疾病中,炎症可能通过激活小胶质细胞和/或星形细胞功能障碍导致大脑结构的变化。为了研究体内炎症和大脑结构变化之间的关系,并探索与精神疾病相关的转录组驱动的功能基础,来自英国伯明翰大学的学者开展了相关研究。

本研究采用多阶段的联系分析,包括孟德尔随机化(MR)、基因表达相关和连接分析。在英国生物库中共有20688名参与者,其中包括临床、基因组和神经影像数据,在艾伦人脑图谱(AHBA)中共有6个来自神经正常人的死后大脑,包括RNA微阵列数据。

暴露因素为循环白细胞介素1(IL-1)、IL-2、IL-6、C反应蛋白(CRP)和脑源性神经营养因子(BDNF)的水平和活性的遗传变异。主要结果是脑成像措施,包括灰质体积(GMV)和皮质厚度(CT)。AHBA数据中的差异基因表达在脑区被映射到MR分析中的重要区域;基因在注释数据库中的生物和疾病过度代表以及在蛋白质-蛋白质相互作用网络中的连接性被测试。

结果显示,在英国生物库样本的20688名参与者中,10828人(52.3%)为女性,平均(SD)年龄为55.5(7.5)岁。在英国生物银行样本中,IL-6的遗传预测水平与中颞皮层(z score, 5.76; P = 8.39 × 10-9)、下颞层(z score, 3.38; P = 7. 20 × 10-5)、纺锤形(z score, 4.70; P = 2.60 × 10-7)和额叶(z score, -3.59; P = 3.30 × 10-5)皮质以及额叶上区的CT(z score, -5.11; P = 3.22 × 10-7)。

多重比较校正后,没有发现IL-1、IL-2、CRP或BDNF的明显关联。全脑共表达分析显示,在颞中回(MTG)优先表达的基因有一个高度互联的网络,它与IL-6进一步形成了一个高度连接的蛋白质-蛋白质相互作用网络(鉴于STRING数据库中相互作用的普遍性和程度,对预期与观察到的网络进行富集测试。观察到的43个节点/30条边与预期的8条边;平均节点度,1.4;全基因组意义,P=4.54×10-9)。MTG差异表达的基因在功能上富集于精神分裂症、自闭症谱系障碍和癫痫的生物过程。
