JAMA Neuro:哪种更有效?老年痴呆8种血浆淀粉样β42/40检测方法的头对头比较

在检测脑Aβ病理时,基于MS的方法比大多数免疫测定的血浆Aβ42/40表现更好。

为了在临床实践中广泛实施对阿尔茨海默病(AD)生物标志物的检测,并在临床试验中促进患者筛选和监测治疗反应,需要对大脑淀粉样β(Aβ)病理学进行基于血液的筛选。

近期,来自瑞典隆德大学马尔默临床科学系临床记忆研究所的专家开展了一项队列研究,旨在比较使用8种不同的Aβ检测方法测量的血浆Aβ42/40在检测早期AD患者脑部Aβ异常状态时的表现。结果发表在近期JAMA Neurology杂志上。

本研究包括来自BioFINDER队列的182名认知无障碍参与者和104名轻度认知障碍(MCI)患者,他们在瑞典的3家不同医院入组,在2010年至2014年期间接受了Aβ正电子发射断层扫描(PET)成像和脑脊液(CSF)及血浆采集。分别使用华盛顿大学开发的免疫沉淀耦合质谱(IP-MS-WashU)、Araclon开发的无抗体液相色谱质谱(LC-MS-Arc)以及罗氏诊断公司(IA-Elc)、Euroimmun(IA-EI)和阿姆斯特丹大学医学中心、ADx Neurosciences和Quanterix(IA-N4PE)的免疫测定法来测量血浆Aβ42/40。还在200名参与者中使用岛津公司的基于IP-MS的方法(IP-MS-Shim),在227名参与者中使用哥德堡大学的基于IP-MS的方法(IP-MS-UGOT)和Quanterix公司的另一种免疫测定法(IA-Quan)测量血浆Aβ42/40。

随后,为了验证,从AD神经影像倡议中纳入了122名参与者(51名认知正常,51名MCI,20名有AD痴呆),使用IP-MS-WashU、IP-MS-Shim、IP-MS-UGOT、IA-Elc、IA-N4PE和IA-Quan检测方法进行了Aβ-PET和血浆Aβ评估。主要结果是评价使用8种不同的检测方法量化的血浆Aβ42/40对CSF Aβ42/40和Aβ-PET状态异常的鉴别准确性。

最终该研究共纳入408名参与者。在BioFINDER队列中,平均(SD)年龄为71.6(5.6)岁, 49.3%为女性。当识别整个队列中CSF Aβ42/40异常的参与者时,血浆IP-MS-WashU Aβ42/40(AUC: 0.86;95% CI, 0.81-0.90)显示出比LC-MS-Arc Aβ42/40、IA-Elc Aβ42/40、IA-EI Aβ42/40和IA-N4PE Aβ42/40(AUC范围: 0.69-0.78)明显更准确。

同时,血浆IP-MS-WashU Aβ42/40的表现明显优于IP-MS-UGOT Aβ42/40和IA-Quan Aβ42/40(AUC分别为:0.84 vs 0.68和0.64),而在有这些生物标志物的2个亚群中,IP-MS-WashU Aβ42/40和IP-MS-Shim Aβ42/40的AUC没有区别(0.87 vs 0.83)。进一步使用Aβ-PET作为结果时结果相似。

此外,血浆IPMS-WashU Aβ42/40和IPMS-Shim Aβ42/40与CSF Aβ42/40的相关系数最高(r=0.56-0.65)。BioFINDER的结果在AD神经影像倡议队列中得到了验证,其中IP-MS-WashU测定的表现明显优于IP-MS-UGOT、IA-Elc、IA-N4PE和IA-Quan测定,但IP-MS-Shim测定没有。

综上,来自2个独立队列的结果表明,在检测脑Aβ病理时,基于MS的方法比大多数免疫测定的血浆Aβ42/40表现更好。
