J Anesth:急性呼吸衰竭患者,无创通气真的有用吗?

该meta分析旨在比较无创通气(NIV)和有创机械通气(INMV)以及常规氧疗(O2)对急性呼吸衰竭(ARF)患者死亡率、气管插管率的影响。

我们检索了MEDLINE、EMBASE和Cochrane临床试验数据库在1949和2015年5月之间使用无创通气的急性呼吸衰竭患者的随机试验。排除因拔管、心源性肺水肿和慢性阻塞性肺疾病引起的急性呼吸衰竭。

这项meta分析共纳入21项研究,1691例患者,其中NIV组846例,对照组(INMV或常规氧疗)845例。结果显示,对照组共有191例患者(22.6%)在治疗期间死亡,对照组有261例(30.9%),短期死亡率(住院期间死亡率)的合并比值比(OR)为0.56(95% CI 0.40-0.78)。常规氧疗组和NIV组相比,短期死亡率分别为155(27.4%) vs 204(36%),此组比较中,短期死亡率的合并OR值为0.56(95% CI 0.36-0.85)。NIV组和INMV组相比,患者短期死亡率分别为36(12.9%) vs 57(20.5%),此组比较中,短期死亡率的合并OR值为0.56(95% CI 0.34-0.90)。NIV组患者需要气管插管术的有106例(22.7%),常规氧疗组183例(39.4%),合并OR值为 0.37(95% CI 0.25-0.55)。分析存在发表偏倚,证据质量较低。

所以我们得出结论,与常规氧疗和有创机械通气相比,无创通气降低了患者短期死亡率,同时降低了合并急性呼吸衰竭患者的气管插管率。
