心肌梗塞持续30秒即可检测到心肌肌钙蛋白升高

心肌肌钙蛋白(cTns)是诊断急性心肌梗死的基石。持续缺血时间需要多久才可以以诱导释放可检测的cTns,或缺血事件后cTns的极早期释放动力学的相关了解仍很有限。Copeptin可能在早期排除心肌梗塞中具有补充作用。

本研究目的是在人类实验性气囊诱导性缺血后的最初几小时内评估cTns和Copeptin的释放情况。

招募了34位受试者(中位年龄 60岁[四分位范围:51-64岁],15位男性),血管造影提示正常冠状动脉,随机分至4组,诱导心肌梗塞持续不同的时间(0、30、60或90s)。在左前降支放置气囊诱导梗塞。诱导梗塞后第一个3个小时每间隔15分钟抽一次血,第二个3小时每隔30分钟抽一次血。检测血液中的hs-cTnT、hs-cTnI(Siemens)、hs-cTnI(Abbott)和Copeptin(免疫荧光)。

不同心肌肌钙蛋白浓度的变化:心肌梗塞持续时间依次为0s、30s、60s、90s;红色:hs-cTnI(Siemens);绿色:hs-cTnI(Abbott);蓝色:hs-cTnT

无患者发生任何并发症。在所有3个实验组中都检测到了cTn水平升高,且cTn的升高量级与梗塞持续时间相关。在90s缺血后15分钟首次检测到hs-cTnI(Siemens)浓度升高(增加中位值:43.7%),并且比其他测定法更陡峭地升高并且具有更高的峰值。Copeptin水平未明显改变。

Copeptin水平变化

根据基线和180分钟时的cTnT、hs-cTnI (Siemens)和hs-cTnI (Abbott)水平,60s组分别对应的有1位(11%)、0位和0位患者符合生化性心肌梗塞的标准,90s组分别对应有5位(63%)、2位(25%)和1位(11%)符合生化性心肌梗塞的标准。

该研究是首个报告人类实验性冠状动脉球囊闭塞后不同时间的cTn的早期释放动力。仅缺血30s后即可检测到cTn升高。以Siemens发检测hs-cTnI升高的更快更高。Copeptin水平未明显改变。
