制药公司NeuroRx今日宣布，在FDA快速通道指定下进行的IIb / III期临床试验已完成60天顶线数据分析，该试验论证了ZYESAMI治疗COVID-19呼吸衰竭患者的有效性。ZYESAMI是一款血管活性肠多肽（VIP）类似物。

血管活性肠多肽（VIP）由已故的Sami Said博士于1970年首次发现。尽管最初在肺中被发现，但现在已知VIP会在整个身体中产生，但主要集中在肺部。在超过100个同行评议的研究中显示，VIP在呼吸窘迫、急性肺损伤和炎症的动物模型中具有抗炎/抗细胞因子活性。最重要的是，VIP与II型肺泡细胞（ATII）特异性结合。VIP刺激ATII细胞生成表面活性剂，该表面活性剂必须覆盖在肺内，以使肺与血液交换氧气。表面活性剂的损失会导致呼吸衰竭和肺泡塌陷，这是COVID-19的标志性事件。

COVID-19相关的呼吸衰竭是由SARS-CoV-2病毒选择性感染ATII细胞引起的。ATII细胞由于其（ACE2）表面受体而容易受到攻击。ATII细胞的冠状病毒感染会关闭表面活性剂的产生，触发炎症性细胞因子的形成，并导致细胞死亡。VIP被证明可上调表面活性剂的产生，阻断ATII细胞中冠状病毒的复制并防止病毒诱导的细胞死亡。
