NEJM:多管齐下治疗镰状细胞状贫血

镰状细胞疾病的典型症状就是镰状细胞危机的反复发作,也叫做血管阻塞危机,在这种情况下病人红细胞形状改变,凝结在一起堵塞小血管,导致疼痛和器官损伤。在新一期《New England Journal of Medicine》的编辑文章中,波士顿大学医学院(BUSM)的研究人员强调为了防止这些有害的状况,应该多管齐下联合多种药物一起治疗镰状细胞疾病。


2017年2月3日讯 /生物谷BIOON /——镰状细胞疾病的典型症状就是镰状细胞危机的反复发作,也叫做血管阻塞危机,在这种情况下病人红细胞形状改变,凝结在一起堵塞小血管,导致疼痛和器官损伤。在新一期《New England Journal of Medicine》的编辑文章中,波士顿大学医学院(BUSM)的研究人员强调为了防止这些有害的状况,应该多管齐下联合多种药物一起治疗镰状细胞疾病。

在该杂志前一期的文章中,研究人员(Ataga等人)报道了一种靶向P-选择素的抗体,P-选择素是一种蛋白质,涉及血管阻塞危机中红细胞和其他细胞的粘附作用。他们发现和对照组相比,有45%注射这种抗体(crizanlizumab)的病人的镰状细胞危机得到减缓。

“目前治疗镰状细胞贫血的药物很有限,我们期待新的有效疗法。”论文作者、BUSM药学教授Martin Steinberg博士说道,“如果这种疗法(或者与之相似的疗法)最终被批准,将显著延长镰状细胞贫血病人的生命。”

Steinberg认为因为目前没有单一疗法可以消除镰状细胞危机,因此涉及靶向该病理过程中不同信号通路中不同靶点的药物的联合疗法也许最终能获得成功。“目前治疗镰状细胞性贫血的药物羟基脲只能帮助大部分人,但是不能帮助所有病人。我们的临床试验表明另一种和羟基脲作用机理不同的药物也有一定疗效,因此可以单独使用或者与羟基脲一起使用以防止严重的病情。”

同时,Steinberg也指出尽管镰状细胞危机减少,但是病人调查问卷检测出的主观临床获益率很低。尽管如此,Steinberg认为使用抗粘附剂的新型治疗方案很有希望最终被批准用于临床。“这是一项很重要的临床研究,临床结果很好,一旦疗效被进一步确认,将为镰状细胞性贫血病人带来重要的新治疗方案。”
