Stroke:西洛他唑和单硝酸异山梨酯对脑血流动力学的影响

脑小血管疾病-卒中和痴呆的主要原因,与脑血管功能障碍相关。目前,没有阻止疾病进展的特定治疗。脑小血管疾病与脑血管反应性降低和脑脊液(CSF)动力学受损有关。单硝酸异山梨酯和西洛他唑可以改善这些脑血管功能障碍,其中,西洛他唑可以减少卒中复发以及认知障碍。

近日,心血管疾病领域权威杂志Stroke上发表了一篇研究文章,研究人员探究了短期单硝酸异山梨酯和西洛他唑单独或联合治疗是否能改善腔隙性缺血性卒中患者的磁共振成像检查测量的脑血管功能。

该研究的参与者被随机分配至单独使用单硝酸异山梨酯治疗、单独使用西洛他唑治疗、同时使用单硝酸异山梨酯和西洛他唑治疗,或不服用任何药物。参与者在基线和用药8周后接受了结构性、脑血管反应性(对6%二氧化碳的反应)和相位对比搏动磁共振成像检查结果。

在27名参与者(平均年龄为68±7.7;44%为女性)中,22名完成了具有完整数据集的脑血管反应性和搏动成像检查。与不服用药物组比较(β=0.021%/mmHg[95%CI为0.005-0.037]),服用单硝酸异山梨酯(β=0.021%/mmHg[95%CI为0.003-0.040])和西洛他唑(β=0.035%/mmHg[95%CI为0.014-0.056])单药治疗组和服用任何药物的患者白质脑血管反应性增加。

由此可见,虽然受到小样本量的限制,但该研究的结果证明在临床试验中使用磁共振成像检查测量脑血管功能是可行的,并且采用单硝酸异山梨酯和西洛他唑治疗可以改善脑血管功能。
