BMJ：爱丁堡产后抑郁量表用于孕产妇抑郁诊断的准确性研究

近日研究人员考察了爱丁堡产后抑郁量表(EPDS)用于孕妇和产后女性抑郁症诊断的准确性。
 
本次研究为个体参与者数据荟萃分析，收集截至2018年10月3日Medline、Medline In-Process、PsycInfo和科学网相关研究资料，符合条件的研究包括EPDS评分和基于有效诊断访谈的抑郁症分类研究。
 
58项研究纳入本次荟萃分析，包含15557名参与者，2069人患有严重抑郁症。EPDS评分的敏感度和特异性均在11或更高的截止值处达到最大值。在采用半结构化访谈为标准的研究中（36项研究，9066名参与者，1330名重度抑郁症患者），当以EPDS评分10或更高作为诊断标准时，EPDS评分的敏感性和特异性分别为0.85和0.84，当以11或更高时为诊断标准时，敏感性和特异性分别为0.81和0.88，以13或更高时为诊断标准时，敏感性和特异性分别为0.66和0.95。在其他参考标准和亚组中，准确度相似。
 
研究认为，爱丁堡产后抑郁量表评估11及以上作为孕妇及产后女性抑郁的诊断标准即可达到最大化的敏感性和特异性，标准继续提高对敏感性的提升不显著，但13分以上后特异性大大提升。
