JCEM:体重指数和血清尿酸与痴呆症之间的关联

BMI和尿酸都是痴呆症的独立预测因子,并且BMI和尿酸与痴呆症之间存在剂量-反应的负向关联。

最近的研究表明较高的BMI和血清尿酸水平与较低的痴呆症风险相关。但是,这些负相关性仍然存在争议,并且血清尿酸和BMI是否相互混淆尚不明确。近日,内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章,该研究旨在调查BMI和尿酸之间的独立关联,以及它们与痴呆症风险之间的相互作用。

研究人员分析了来自英国生物库的502528名37岁至73岁的参与者,这些参与者在2006年至2010年之间记录了BMI和尿酸水平。参与者的痴呆症情况通过使用电子健康记录进行确定。

在平均8.1年的随访期间,共有2138名参与者患了痴呆症。与体重正常的参与者相比,体重不足的参与者患痴呆症的风险增加(风险比[HR]=1.91,95%置信区间[CI]:1.24-2.97)。但是,随着体重的增加,痴呆症的风险继续下降,与正常体重的参与者相比,超重和肥胖的参与者患痴呆症的风险分别降低19%(HR=0.81,95%:0.73-0.90)和22%(HR=0.78,95%CI:0.68-0.88)。尿酸水平处于最低五分位数的参与者相比,尿酸水平处于最高五分位数的参与者患痴呆症的风险也降低了25%(HR=0.75,95%CI:0.64-0.87)。BMI和尿酸盐与痴呆症之间存在显著的相互作用(相互作用P=0.004),肥胖增强了血清尿酸对痴呆症风险的保护作用。

由此可见,BMI和尿酸都是痴呆症的独立预测因子,并且BMI和尿酸与痴呆症之间存在剂量-反应的负向关联。
