研究是一个单中心回顾性审查。包括了接受PN/RN(1999-2008,n=1605)的肾癌患者(T1-T3a/N0/M0)。年龄中位数为60岁,64%的患者为男性。并存症包括高血压(60%)、血管疾病(19%)、糖尿病(21%)和慢性肾脏疾病(22%)。954名患者(59%)进行了PN。术前中位eGFR和NB-GFR分别为80和60ml/min/1.73 m2。肿瘤中位直径为3.6 cm(四分位数范围[IQR]=2.4,5.5);70%的肿瘤为透明细胞,40%为高等级。病理显示pT1-2/N0/M0和pT3a/N0/M0分别占81%和19%。幸存者的随访中位时间为11.5年(IQR=4,14)。10年时癌症特异性生存率、无复发生存率和总生存率分别为94%、88%和73%。在多变量分析中,年龄增加(HR=1.03,p=0.04)、肿瘤大小增加(HR=1.24,p<0.01)、肿瘤3/4级(HR=3.17,p<0.01)和透明细胞组织学(HR=2.92,p<0.01)与CSM的风险性增加相关。术前eGFR或NB-GFR均与CSM或CR不相关(均P>0.1),而术前eGFR增加与ACM风险降低相关(HR=0.87,P<0.01)。局限性包括回顾性设计和潜在的选择偏差。
