脑微出血患者的复发性中风、缺血性中风、脑出血和死亡率的风险增加

据报道,脑微出血可导致复发性中风和脑出血风险增加,因此中风和磁共振成像出现微出血患者是否需接受抗血栓治疗呢?近日研究人员考察了对于不明来源栓塞性中风(ESUS)微出血患者,接受抗凝与抗血小板治疗的效果。
NAVIGATE ESUS研究为一项国际,双盲,随机,事件驱动的III期临床研究,在31个国家的459个中风中心开展。年龄在50岁或50岁以上的患者,在筛查前7天至6个月有神经影像学证实的ESUS,3699名患者在基线临床磁共振成像中报告了脑微出血,随机每天接受15mg利伐沙班或100mg阿司匹林。研究的主要终点为复发性中风,次要结果是缺血性中风、脑出血和全因死亡率。
 
脑微出血患者的平均年龄为69.5岁,男性241人(61%),白种人201人(51%)。高龄(比值比[OR]每年1.03)、东亚种族(OR:1.57)、高血压(OR:2.20)、多区域梗塞(OR:1.95)、慢性梗塞(OR:1.78)和隐匿性脑出血(OR:5.23)与微出血独立相关。微出血的存在导致复发性中风风险增加1.5倍(危险比[HR]:1.5)、脑出血风险增加4倍(HR:4.2)、全因死亡风险增加2倍(HR:2.1),严格意义上的大叶微出血导致缺血性中风风险增加2.5倍(HR:2.3)。对于存在微出血患者,利伐沙班治疗后的脑出血风险与阿司匹林相当(HR:3.1 vs 3.0)。
 
研究认为,脑微出血患者的复发性中风、缺血性中风、脑出血和死亡率的风险增加。
