防治骨质流失,这些“小药物”要科学使用!

概  述

 

急性骨丢失是指因病情需要制动数日至数周,甚至更长时间,当某部位或全身的活动功能受限时,致负钙平衡,破骨细胞活性增加,溶骨速度大于成骨速度,且其活性大于成骨细胞,易引起局部和全身骨量快速下降,其常与应力刺激减少(废用)、炎症及骨代谢调节激素变化甲状旁腺激素(PTH)分泌增加相关

 

《骨科急性骨丢失防治专家共识》(2021年)中指出,急性骨丢失一般发生在急性制动或卧床后6-8周,骨量急剧下降到最低水平,是预期骨折风险最高的时期,后会出现骨量的逐步恢复,但骨形成和骨吸收的速率不平衡,骨量虽较急性骨丢失阶段有所上升,但仍处于骨量丢失的过程中,这个恢复时期持续2-24个月,称进行性骨丢失,预期骨折风险仍然很高。经历急性和进行性骨丢失后,大量骨质流失,骨骼强度降低,且往往多年无法完全恢复到骨折前的骨量水平,增加骨质疏松、骨折固定、关节置换失败等风险。

 

急性骨丢失常见病因有骨折、创伤等引起的急性制动或石膏固定;髋关节置换术、膝关节置换术引起的负重减少与假体周围炎症;失重、脊髓损伤;椎间盘病变的保守治疗及各种原因的骨科牵引等致的长期卧床。

 

《骨科急性骨丢失防治专家共识》(2021年)中指出,骨质疏松者急性骨丢失,除常规的骨科治疗外,尽早开展功能康复,阻止急性骨丢失,增加骨量,治疗骨质疏松,避免骨量的进一步丢失,降低预期骨折或受伤的风险;非骨质疏松者急性骨丢失的防治主要为保持健康的生活方式、注意缓解骨痛,早期功能康复、药物干预、增加骨量,急性骨丢失治疗时也要注意进行性骨丢失的防治,以尽可能缩小恢复到正常峰值骨量的难度,尽可能恢复到损伤前的峰值骨量。此外,骨痛是骨量减少、骨质疏松的一个重要症状,迅速有效地止痛,可避免长时间废用造成持续性骨丢失。

 

急性骨丢失用药

 

(一)钙剂与维生素D

 

钙剂和维生素D是急性骨丢失防治的基础措施,也是抗骨质疏松症的基础治疗药物,可促进骨形成与骨骼矿化、降低骨折风险。

 

对骨质疏松者的快速骨丢失,补钙剂量成人不少于1000mg/d,并长期持续服用,同时补充维生素D,补充剂量为800-1200IU/d。对非骨质疏松者的快速骨丢失,补钙剂量成人800-1000mg/d,持续时间至少半年以上,同时补充维生素D,补充剂量为800-1200IU/d。

 

注意事项:高钙血症及高尿钙症者避免使用钙剂,以避免增加肾结石和血管疾病风险。维生素D可增加肠钙吸收,建议定期监测血钙和尿钙水平,以防引起高钙血症。

 

(二)抗骨质疏松症药物

 

骨质疏松症者急性骨丢失,早期药物干预急性骨丢失,严重时可与抗骨质疏松症药物联合治疗,骨丢失严重时可联用不同作用机制的抗骨质疏松症药物。

 

抗骨质疏松症药物主要有抗骨吸收药物(双膦酸盐类药物、雌激素类药物、选择性雌激素受体调节剂、RANKL抑制剂等)、促进骨形成药物(甲状旁腺激素类似物)等。

 

 

(三)降钙素类药物

 

如鳗鱼降钙素类似物(依降钙素)、鲑鱼降钙素,为抗骨吸收药物,可抑制破骨细胞的生物活性和增生、减少破骨细胞数量、抑制骨吸收、减少骨量丢失及增加骨量,并特异性结合受体,快速升高β-内啡肽水平,抑制前列腺素等致痛物质的合成,多种途径发挥快速、强效镇痛作用,故而明显缓解骨痛,对骨质疏松症及其骨折引起的骨痛有效,可用于急性骨丢失、进行性骨丢失、男性骨质疏松症、女性绝经后骨质疏松症、女性围绝经期骨质疏松症、继发性骨质疏松症、骨痛的治疗。

 

中至重度骨痛的骨质疏松症者,或骨折围手术期者,建议使用降钙素类药物缓解骨质疏松性骨痛。此外,国内外指南均提到鲑降钙素可阻止急性骨丢失,缓解骨质疏松及其骨折后的骨痛,建议在骨质疏松的制动者中使用,必要时可采用间歇性重复给药。

 

注意事项:不良反应有面部潮红、恶心等,偶有过敏现象。有潜在增加肿瘤风险的可能,连续使用一般不超过3个月。

 

(四)避免使用致骨丢失的药物

 

影响骨代谢的药物包括抗凝剂、糖皮质激素、质子泵抑制剂、强效利尿剂(如呋塞米等)、抗病毒药物(如利托那韦等蛋白酶抑制剂)、噻唑烷二酮类药物、抗癫痫药物、芳香化酶抑制剂(如阿那曲唑等)、肿瘤化疗药物、铝剂(抑酸剂)、选择性5-羟色胺再摄取抑制剂、钙调磷酸酶抑制剂、促性腺激素释放激素类似物、甲状腺激素(过量)、避孕剂等。
