Crit Care:重症监护病房生命体征昼夜节律的意义

ICU患者入住期间生命体征的昼夜节律仍然存在,但对这些节律的定量评估可能会为ICU患者提供预后信息。

重症监护病房(ICU)患者生命体征昼夜节律紊乱与诸如免疫系统紊乱、谵妄和死亡率增加等并发症相关。但是,这种紊乱的发生率和程度尚未明确。目前,对其进行检测的工具也十分有限。

近日,危重病医学领域权威杂志Critical Care上发表了一篇研究文章,该系统评价评估并比较了生命体征的昼夜节律,包括收缩压、心率、呼吸频率和体温。

研究人员在三个大型的可公开获得的临床数据库中,比较了从ICU康复的患者和从ICU未康复的患者(死亡或转诊至护理机构)。这项比较包括对节律特征的定性评估,以及对例如高峰-最低点偏移等指标的定量指标,并与人口统计学匹配的“恢复”特征进行了相关性评估。

在ICU住院期间,所有生命体征仍存在昼夜节律改变。与没有从ICU中康复的患者相比,在从ICU康复患者在整个ICU住院期间,峰谷波动和与“恢复”特征的相关性通常更大。

由此可见,这些结果表明,ICU患者入住期间生命体征的昼夜节律仍然存在,但对这些节律的定量评估可能会为ICU患者提供预后信息
