新发现的PRIME细胞不仅可以提供新的见解,从而可以更好地预测何时会发生严重的疼痛和肿胀,还可以为类风湿关节炎的根本原因提供新的见解,并可能为治疗甚至预防耀斑提供新的途径从发生。这将使耀斑的破坏性降低,更易于管理。该研究发表在《New England Journal of Medicine》上,题为“RNA Identification of PRIME Cells Predicting Rheumatoid Arthritis Flares”。

使用这种方法需要大量的数据。类风湿关节炎患者通常会去医院每月进行一次血液检查,但科学家们希望了解血液基因表达模式每周如何变化。因此,他们没有让病人每周都来,而是开发了一种手指刺工具包,用于在家中收集样本。这种优化方法,使患者自己可以获取指尖血液样本进行RNA测序(RNA-seq),以方便每周进行数月至数年的血液采样。

研究组为类风湿关节炎患者建立了一种临床技术方案,用于回家后多次采血,以进行纵向RNA测序(RNA-seq)。指标患者在4年的时间里,8次耀斑期间,采集了364个时间点的样本,另外3名患者采集了235个时间点的样本。研究组鉴定了在耀斑前差异表达的转录本,并将其与滑膜单细胞RNA序列数据进行了比较。用流式细胞仪和分类血细胞RNA序列对其他患者进行验证。

类风湿关节炎发作前1-2周,在血液转录谱中观察到一致的变化。B细胞活化后,类风湿关节炎患者血液中循环的CD45-CD31-PDPN +炎症前间充质细胞(PRIME)扩增。这些细胞具有炎性滑膜成纤维细胞的特征。所有4例患者在耀斑期间循环PRIME细胞的水平均下降,流式细胞仪和分类细胞RNA-seq证实了另外19名类风湿性关节炎患者中存在PRIME细胞。

在四年的时间里,患者将血液样本邮寄到实验室,并报告了症状,并指出了爆发的时间。研究人员分析了症状恶化前几周收集的血液。从纵向上收集样本可以搜索临床症状之前的转录特征,并将这些血液RNA谱与滑膜单细胞RNA-seq(scRNA-seq)的数据进行比较,以确定在生物学上连贯的转录本是否可识别。他们在症状发作之前以及患者开始出现症状时先抽血。


最初的观察并不完全令人惊讶。在爆发前两周收集的样本中,研究人员发现免疫系统B细胞有所增加。这并不是不可预见的,因为研究人员已经知道这些细胞会袭击类风湿关节炎患者的关节。

类风湿关节炎导致关节疼痛和发炎。B细胞(此处显示一个)在疾病中起作用。这些细胞可以激活PRIME细胞,PRIME细胞是在症状发作之前出现在血液中的新发现的细胞。

但是在爆发前几天收集的样本中,科学家们注意到RNA的增加与任何已知类型的血液或免疫细胞的遗传特征都不匹配。研究者认为这是事件的关键点。

这些细胞看起来不像正常情况下在血液中会发现的。相反,RNA特征类似于骨骼、软骨或肌肉细胞的特征。作者发现,在耀斑爆发之前表达的基因通常在骨骼、肌肉和细胞外基质中活跃,这是在血细胞中发现的奇怪途径,这也激起了作者的兴趣。

这些被称为PRIME细胞的新细胞通常仅以低水平存在于血液中,但随后在爆发期的前一周出现峰值,而在爆发期来临时几乎消失了。类风湿关节炎耀斑的纵向基因组分析揭示了耀斑爆发前血液中的PRIME细胞,并提出了一种模型,其中这些细胞在耀斑爆发前的几周被B细胞激活,随后从血液中迁移到滑膜中。

PRIME细胞的功能尚待确定,但科学家们已经有了一些线索。基于RNA表达谱,他们发现细胞看起来与滑膜成纤维细胞非常相似,滑膜成纤维细胞存在于关节的组织内层。作者认为PRIME细胞可能是这些成纤维细胞的前体,已知成纤维细胞在引起类风湿性关节炎症状中起作用。例如,在小鼠实验中,科学家将发炎的关节中的成纤维细胞移植到了健康的关节中,受体关节后来也变成了关节炎。综上所述,该观察结果提出了一个模型,其中B细胞的顺序激活会在爆发前激活PRIME细胞。然后,在发炎的滑膜发炎期间,这些细胞就很明显地表现为炎性的成纤维细胞。

科学家们现在正在努力尽可能多地了解PRIME细胞,以及如何快速检测它们。如果医生可以轻松地检测血液中是否存在PRIME细胞,那么他们就可以警告患者发生耀斑的可能性。而且,如果发现PRIME细胞在引起耀斑中起作用,这一发现也可能为开发可以减轻炎症的疗法打开大门。

研究小组的策略还可以用于研究其他炎症性疾病的起伏。这种不断变化的临床过程是许多自身免疫性疾病的特征,包括多发性硬化症,系统性红斑狼疮和炎症性肠病,强调需要开发一种方法来了解触发从静止状态过渡到自身免疫性耀斑的因素。

研究小组的下一步措施之一是评估更多的患者,这些PRIME细胞的存在是否可以再现性地预测耀斑。研究人员仍在招募患者进行这项研究。目前,该小组的采血系统仅可用于研究。作者还希望研究PRIME细胞的分子特征。

该研究小组在爆发之前识别出与髓样细胞,嗜中性粒细胞和血小板相关的基因表达特征,以及在爆发前2周采集的样本中与幼稚B细胞相关的另一个特征。有趣的是,在耀斑发作前1周采集的样本中强烈表达了基因表达特征,这让人联想到间充质细胞。

“我们将这种间充质标记与先前由加速医学合作组织收集的滑膜单细胞RNA测序数据进行了比较,发现该基因标记与滑膜下成纤维细胞的基因标记重叠,这表明与滑膜成纤维细胞相似的细胞可能在血。”奥兰治说。

研究人员将这一理论再向前迈进了一步,他们在另外19名RA患者的血液样本中寻找了这些成纤维样细胞。他们检测到的细胞是CD31–CD45–PDPN +,在RA患者中比在健康个体中更常见。对分选的血液CD31–CD45–PDPN +细胞进行RNA测序,发现爆发前1周上调的基因和滑膜成纤维细胞中表达的基因富集,从而使研究人员将这些细胞命名为炎症前间充质(PRIME)细胞。

“在RA发作之前在血液中发现PRIME细胞特别令人感兴趣,因为其他研究表明,RA滑膜成纤维细胞可以传播关节炎,并在动物模型中充当滑膜炎性浸润的关键看门人,” Orange断言。

由于只有4名患者参与了这项研究的长期监测,因此需要进行进一步的研究来确定PRIME细胞是否在所有RA患者以及所有RA患者中均在疾病发作之前。奥兰治及其同事还希望研究在爆发前两周出现的B细胞激活或募集PRIME细胞的可能机制。
