Crit Care:与通过外周静脉输注血管加压药相关的不良事件

与PIV升压药相关的不良事件发生率较低。需要进行额外的研究来评估PIV位置和大小,升压药类型和剂量以及患者特征对PIV升压药安全性的影响。

通过外周静脉输注(PIV)是否可以安全地输注血管加压药尚未明确。

近日,危重病医学领域权威杂志Critical Care上发表了一篇研究文章,研究人员旨在通过系统评价和荟萃分析评估在任何急性护理环境中任何年龄段患者与PIV升压药给药相关的局部解剖学不良事件的发生率。

研究人员检索了MEDLINE、EMBASE、CINAHL、Cochrane中央对照试验登记中心,检索时间从数据库成立到2019年10月。研究人员还检索了包含研究和相关评价的参考文献以及相关的会议论文集。研究纳入标准为:(1)队列、准实验研究或随机对照试验研究设计;(2)在任何年龄或临床环境的患者;(3)报告了与PIV升压药给药相关的局部解剖学不良事件。研究人员使用修订的Cochrane偏见风险工具对随机试验进行偏见风险评估。研究人员使用随机效应荟萃分析汇总了发病率的估计值,并通过亚组分析探索了异质性的来源。

该系统评价纳入了23项研究,其中分别有16项和7项研究描述了成年人和儿童。来自11项成人研究的荟萃分析纳入了16055名患者,显示与PIV升压药相关的不良事件合并发生率为1.8%(95%CI为0.1-4.8%,I2=93.7%)。在儿童研究中,来自4项研究和388名患儿的荟萃分析显示,不良事件的总发生率为3.3%(95%CI为0.0-10.1%,I2=82.4%)。根据临床试验的位置、研究之间偏倚或设计的风险、PIV位置和大小或升压药类型或持续时间的差异,亚组分析未检测到与分层相关的统计学上显著的效应。

由此可见,与PIV升压药相关的不良事件发生率较低。需要进行额外的研究来评估PIV位置和大小,升压药类型和剂量以及患者特征对PIV升压药安全性的影响。
