无症状呼吸个体中约有10%的患者表现为ILA。血清中促炎分子和MMP浓度升高与ILA相关。

50岁以上的人群中约有8-10%的个体出现间质性肺部异常(ILA),但其危险因素尚未明确。

近日,呼吸疾病领域权威杂志Eur Respir J上发表了一篇研究文章,研究人员从“肺衰老计划”中招募了817名参与者,80名(9.7%)参与者伴有ILA,并与HRCT正常的同一队列的564名参与者进行了比较,以评估人口统计学和功能差异,并从同一队列中随机选择80名参与者检测生物标志物。研究人员评估了参与者MUC5B(rs35705950)突变、端粒长度和血清中基质金属蛋白酶(MMP)-1、MMP-2、MMP-3、MMP-7、MMP-8、MMP-9、MMP-12、MMP-13、白介素(IL)-6的水平、表面活性蛋白(SP)-D、α-Klotho和抵抗素。

ILA患者通常为男性(p<0.005),年龄大于对照组(p<0.0001),多为吸烟者(p=0.01),且MUC5B突变rs35705950 出现频率较高(OR为3.5;95%CI为1.29-9.44,p=0.01),并降低了DLCO和氧饱和度。ILA患者的抵抗素、IL-6、SP-D、MMP-1、MMP-7和MMP-13水平显著增加。抵抗素(12±5 ng/ml vs. 9±4 ng/ml;p=0.0005)和MMP-13(357±143 vs. 298±116pg/ml,p=0.004)是升高最明显的生物标志物。随访(24±18个月)时,18例患者出现病情进展,与胃食管反流病(OR为4.1,95%CI为1.2-12.9,p=0.02)、女性糖尿病(OR为5.3,95%CI为1.03-27.4,p=0.01)相关。
