Clin Nutrition:鸡蛋和鸡蛋来源的胆固醇消耗与死亡率的关系

目前,将鸡蛋和饮食中胆固醇的摄入与人类健康和长寿联系起来的文献证据备受争议,来自非西方人群的数据更是少的可怜。因此,本项研究的目的是前瞻性地评估中国人群中鸡蛋和胆固醇的消耗与死亡率的关系。

背景及目标:

目前,将鸡蛋和饮食中胆固醇的摄入与人类健康和长寿联系起来的文献证据备受争议,来自非西方人群的数据更是少的可怜。因此,本项研究的目的是前瞻性地评估中国人群中鸡蛋和胆固醇的消耗与死亡率的关系。

 

 

方法

研究人员追踪了1991年至2015年间来自中国健康与营养调查(CHNS)研究的18914名≥20岁的成年人数据。在每个调查年中,研究人员均使用3天24小时饮食记录来收集饮食数据,计算出鸡蛋和胆固醇摄入量的累计平均值。使用Cox比例风险模型估算了风险比(HRs)和95%置信区间(CIs)。

 

 

结果

在平均随访15年后,据报告总共有1429例死亡病例。鸡蛋的消耗与总死亡率的降低显着相关(P<0.001)。多变量调整后1-3个鸡蛋/周的HR(95%CI)为0.69(0.59–0.81),3–7个鸡蛋/周为0.68(0.59–0.79), ≥2个鸡蛋/天则为0.64(0.52-0.78),而≤1个鸡蛋/周增明显增加死亡率。尽管饮食中的总胆固醇与死亡率无关,但鸡蛋中胆固醇的摄入与总死亡率成反比(P= 0.001),而非蛋源胆固醇的摄入与总死亡率呈正相关(P= 0.01)。用50 g / d鸡蛋代替等量的乳制品,坚果/豆类或红肉,分别使总死亡率降低9%,7%或8%。

 

 

结论

鸡蛋消费与中国人口总死亡率降低有关。从非鸡蛋来源摄取胆固醇可能有害于长寿。中国饮食指南应继续建议习惯性地食用鸡蛋,同时不鼓励从非鸡蛋来源摄入过多的胆固醇。
