EHJ:“撕心裂肺”的痛有了新进展——我国学者发现主动脉瘤标志物

血浆琥珀酸水平较高的个体出现主动脉疾病的风险显著增加

主动脉瘤是猝死的最常见原因之一(特别是60岁以上的男性),因此常被称为“无声杀手”。患者多数在动脉瘤破裂前都不会出现任何征状,故导致高达75%的发病率。

主动脉夹层则是指主动脉腔内的血液从主动脉内膜撕裂处进入主动脉中膜,使中膜分离,沿主动脉长轴方向扩展形成主动脉壁的真假两腔分离状态。本病发病率较低,但高达65%~70%的患者会在急性期死于心脏压塞、心律失常等,故早期诊断和治疗非常必要。

主动脉瘤和夹层(AAD)是高风险的血管疾病,没有有效的治疗方法。巨噬细胞在AAD的发展中起着重要作用。由于琥珀酸盐会引发巨噬细胞的炎症变化,来自我国北京大学基础医学院心血管研究所研究了琥珀酸盐在AAD发病机制中的意义及其临床意义,结果发表在近期European Heart Journal杂志上。

研究人员使用非靶向代谢组学和质谱技术分别测定了发现组和验证组中40人和1665人的血浆琥珀酸浓度。三个不同的小鼠AAD模型被用来确定琥珀酸在AAD发展中的作用。进一步研究了在巨噬细胞介导的炎症背景下,氧化谷氨酸脱氢酶(OGDH)及其转录因子环腺苷单磷酸反应元件结合蛋白1(CREB)的作用,并建立了p38αMKOApoe/-小鼠。

结果显示,琥珀酸水平较高的个体出现主动脉疾病的风险显著增加。随后该结果在验证队列中得到了证实。与健康对照组、急性心肌梗塞(AMI)患者和肺栓塞(PE)患者相比,AAD患者的血浆琥珀酸浓度更高。此外,琥珀酸盐的使用会加重血管紧张素II诱导的AAD和小鼠的血管炎症。

相反,敲除OGDH可减少巨噬细胞中炎症因子的表达。p38α的条件性缺失降低了CREB的磷酸化、OGDH的表达和琥珀酸盐的浓度。巨噬细胞中p38α的条件性缺失减少了血管紧张素II引起的AAD。

综上,血浆琥珀酸水平较高的个体出现主动脉疾病的风险显著增加,且琥珀酸盐的浓度由巨噬细胞中的p38α-CREB-OGDH轴调节。
