Eur Radiol:如何判断肺癌有没有侵犯脏层胸膜?

肺胸膜浸润(VPI)是调整肺癌病理T分类和T1、T2肿瘤组织学类型后的主要不良预后指标之一。因此,术前评估肺癌VPI的存在对选择手术范围非常重要。

    肺胸膜浸润(VPI)是调整肺癌病理T分类和T1、T2肿瘤组织学类型后的主要不良预后指标之一。因此,术前评估肺癌VPI的存在对选择手术范围非常重要。由于VPI与淋巴结转移和局部复发相关,因此尽管是IA期肺癌,若病灶同时伴有VPI,切除范围则由肺段切除术转为肺叶切除术合并纵隔淋巴结清扫。

    但目前,术前基于影像学特征很难准确诊断VPI。CT表现如肿瘤与胸膜直接接触、胸膜牵拉、胸膜收缩等被认为是VPI的可疑影像学表现,但据部分研究表明这些表现的诊断准确率仅在71 - 95%之间。在这种情况下,深度学习能够自动从原始数据中学习有用的表示和特征,而不需要人工介入,这可能会带来诊断上的突破。深度学习在几种影像学模型中表现出比医生更高的诊断准确性。


    近日,发表在European Radiology杂志的一项研究开发并验证了一种术前基于CT的深度学习模型,用于预测早期肺癌的VPI,同时也比较了模型和放射科医师之间的诊断性能,并分析模了型输出的独立诊断价值。

    在本项回顾性研究中,数据集1(用于训练、调优和内部验证)纳入了676例在2009年至2015年之间手行术切除的临床IA期肺腺癌患者。数据集2(用于时间验证)纳入了141例在2017年至2018年期间手术切除的临床I期肺腺癌患者。建立了一种预测VPI的基于CT的深度学习模型,并在区分和校准方面进行了验证。同时进行了观察者表现研究和多元回归分析。

    该模型的受试者工作特性曲线(AUC)下的面积为0.75(95%CI,0.67-0.84),与经委员会认证的放射科医生的曲线下面积(AUC,0.73-0.79;所有p> 0.05)相当。该模型具有比放射科医师更高标准化部分的AUC,特异性范围90%至100%(所有p <0.05)。高灵敏度截断值为0.245时的敏感性为93.8%,特异性为31.2%,高特异度截断值为0.448时的敏感性为47.9%,特异性为86.0%。三位放射科医师中有两位提供了高敏感性(93.8%和97.9%)而非特异性(48.4%和40.9%)诊断。该模型显示了良好的校准(p > 0.05),其输出是VPI的独立预测因子(校正优势比,1.07;95%置信区间,1.03 -1.11;p < 0.001)。

    该基于术前CT的深度学习模型为手术适应患者提取了独立的VPI诊断信息,并表现出专家级的诊断性能。
