近期,有研究人员回顾了目前关于无细胞DNA(cfDNA)和循环肿瘤DNA(ctDNA)的数据,总结了它们在膀胱癌和肾癌的诊断、监测和预后中的作用

他们对2020年8月15日之前的文献进行了回顾,包括前瞻性和回顾性的研究。关键词包括 "无细胞DNA"、"循环肿瘤DNA"、"肾癌"、"肾细胞癌"、"膀胱癌"、"上尿路上皮癌 "和 "尿路上皮癌"。

结果表明,尿液肿瘤DNA(utDNA)检测膀胱癌的敏感性为91%,特异性为96%,优于膀胱镜和细胞学检查。utDNA和ctDNA的增加与非肌层浸润性疾病向肌层浸润性疾病的发展有关。在接受膀胱切除术的患者中,ctDNA与较差的总生存率和疾病复发有关;在接受新辅助化疗的患者中,ctDNA与手术病理上的持续肿瘤有关。 肾癌患者的cfDNA明显高于健康对照组或良性病变患者,可检测的ctDNA和cfDNA的增加与生存率下降有关。

因此,对小型研究的数据综合证明了cfDNA和ctDNA可能具有检测、监测和预后膀胱癌、上尿路尿道癌和肾癌患者的能力
