Respir Res:中性粒细胞弹性蛋白酶作为COPD中细菌感染的生物标记物

慢性阻塞性肺病(COPD)主要与嗜中性粒细胞炎症有关。活性中性粒细胞弹性蛋白酶(NE)是一种丝氨酸蛋白酶,由中性粒细胞分泌,以应对炎症和病原体入侵。本研究旨在探究NE是否可以作为COPD患者的细菌感染

慢性阻塞性肺病(COPD)主要与嗜中性粒细胞炎症有关。活性中性粒细胞弹性蛋白酶(NE)是一种丝氨酸蛋白酶,由中性粒细胞分泌,以应对炎症和病原体入侵。本研究旨在探究NE是否可以作为COPD患者的细菌感染的生物标志物。

 

我们使用ProAxsis公司的ProteaseTag®活性NE免疫测定仪(ProAxsis, Belfast)对慢性阻塞性肺病患者在稳定状态、病情加重和治疗后2周的痰液中的NE进行了定量检测。

 

对30名慢性阻塞性肺病患者(18名男性)的90份样本进行了NE测定,平均年龄(范围)为65(45-81)岁,平均(SD)FEV1为47%(18)。在稳定状态下,NE的几何平均值(95%CI)为2454 ng/mL(1460~4125 ng/mL)。恶化时NE水平显著增加(p = 0.003),细菌相关的恶化时NE水平较高(NE对数差3.873,95%CI对数差1.396~10.740,p = 0.011)。NE是细菌相关加重的准确预测因子(接受者特征曲线下的面积(95%CI)0.812(0.657~0.968))。

 

这些结果表明,在慢性阻塞性肺疾病恶化期间,NE会升高。NE可能是鉴别COPD患者细菌性加重的可行生物标志物。
