心肌梗死的未来风险通常通过心血管风险评分、冠状动脉钙评分或冠状动脉狭窄严重程度来评估。Williams等评估了冠状动脉CT血管造影(CCTA)中未钙化的低衰减斑块负荷是否可能更好地预测心肌梗死的未来风险。

在一项CCTA对稳定胸痛患者的多中心随机对照试验的事后分析中,研究人员评估了致死性或非致死性心肌梗死的未来风险与低衰减斑块负荷(斑块占血管体积的百分比)、心血管风险评分、冠状动脉钙化评分或阻塞性冠状动脉狭窄之间的关系。

共1769位患者(56% 男性,58±10岁),中位随访4.7年,低衰减斑块负荷与心血管风险评分的相关性较弱,与冠状动脉钙评分的相关性较强,与冠状动脉狭窄严重程度相关性非常强。与无致死性或非致死性心肌梗死的患者相比,41位发生致死性或非致死性心肌梗死的患者的低衰减斑块负荷(4.1% vs 7.5%)、冠状动脉钙评分(19 vs 336)和阻塞性冠状动脉狭窄的发生(25% vs 54%)均更高。低衰减斑块负荷是独立于心血管风险评分、冠状动脉钙化评分及冠状动脉狭窄之外的心肌梗死的最强预测因子。低衰减斑块负荷>4%的患者发生继发心肌梗死的可能性是正常人的近5倍。

对于稳定性胸痛的患者,低衰减斑块负荷是致死性或非致死性心肌梗死的最强预测因子。本研究结果挑战了当前经典的心肌梗死风险预测指标的优越性,包括狭窄的严重程度。
