BMJ:AD患者Aβ水平与认知改善的相关性研究

现有试验报告淀粉样蛋白水平降低和认知能力改变的综合证据表明,淀粉样蛋白减少策略并不能显著改善AD患者认知能力

有学者认为,淀粉样斑块和寡聚体是导致阿尔茨海默病患者认知功能下降的病理因素。根据这一假说,许多预防或治疗阿尔茨海默病疗法的主要目的是降低大脑中Aβ水平,但是淀粉样蛋白介导神经元病理的机制目前尚不清楚,并且尚无任何抗淀粉样蛋白治疗方案获得FDA批准,导致淀粉样蛋白理论受到质疑。
近日研究人员开展系统综述及荟萃分析,考察了在涉及淀粉样蛋白药物的临床试验中,淀粉样蛋白水平的降低是否有可能改善认知能力。
 
研究人员从clinicaltrials.gov收集了14项针对淀粉样蛋白机制的阿尔茨海默病预防或治疗药物的随机对照试验,试验的入组标准各不相同,但通常为50岁或以上的成年参与者,诊断为轻度认知障碍或阿尔茨海默病,基线时淀粉样蛋白阳性。研究的主要终点为淀粉样正电子发射断层扫描(PET)脑淀粉样蛋白水平变化与认知改善。
 
14个随机对照试验的汇总结果比任何单一试验的结果更为精确。总体上,患者脑淀粉样蛋白水平摄取值比值单位降低0.1个标准单位,简易精神状态评价量表(MMSE) 得分仅改善0.03,不足以证明脑淀粉样蛋白水平与认知改善的相关性。
 
现有试验报告淀粉样蛋白水平降低和认知能力改变的综合证据表明,淀粉样蛋白减少策略并不能显著改善AD患者认知能力。
