JGAM:接受阿奇霉素治疗的肺炎支原体肺炎儿童肠道菌群组成发生改变

基因测序数据揭示了阿奇霉素治疗肺炎支原体肺炎患儿肠道菌群组成的改变。微生物表达的改变与临床特征相关,这可能有助于诊断和治疗该病。

肺炎支原体是引起儿童社区获得性肺炎最重要的病原菌之一,肺炎支原体感染的致病机制复杂。阿奇霉素是治疗获得性下呼吸道感染和泌尿生殖道感染的有效药物,不良反应轻微。The Journal of General and Applied Microbiology杂志发表了一项研究,旨在比较肺炎支原体肺炎患儿阿奇霉素干预前(PP1)和干预后(PP2)肠道菌群变化,结合体液生化分析,确定肠道菌群对疾病进展的影响。

研究招募15例肺炎支原体肺炎患儿,年龄3~10岁。1~3个月内完成健康和饮食习惯调查。在每个患者入院时收集第一批样本,包括粪便、血液和尿液,第二批样本在1个月后收集。使用QIAamp快速DNA粪便迷你试剂盒(QIAGEN)从每个粪便样本中提取基因组DNA。测定DNA含量。

纳入15例肺炎支原体肺炎患儿接受标准治疗(阿奇霉素,每天10 mg/kg体重),3天为1个疗程。所有参与者都连续完成了2个疗程。平均住院时间为8.2±1.97天。

对OUT物种进行分类。结果表明,两组在门、类、目、科、属、和种上存在差异。PP1组的嗜血杆菌属、巴斯德菌目、巴斯德菌科的丰度显著高于PP2组。

皮尔森相关分析表明,微生物与临床特征强烈相关。PP1中较高的中性粒细胞百分比、较低的淋巴细胞百分比和血小板临界值与肠杆菌科、梭菌纲、γ变形杆菌纲的表达丰度呈正相关,而淋巴细胞数量与三种菌株的丰度呈负相关。副流感嗜血杆菌丰度与超敏C反应蛋白水平及单核细胞数呈正相关。巴斯德菌目、巴斯德菌科的丰度与血红蛋白和红细胞压积呈负相关。
