呼吸道合胞病毒(RSV)不仅可导致婴儿反复喘息,而且可能诱发潜在的特应性哮喘。

为了验证RSV感染对随后的特应性哮喘发展的影响,我们评估了帕利珠单抗(一种抗RSV单克隆抗体)是否通过预防1岁内重度RSV疾病,从而影响随后6年内复发性喘息和特应性哮喘的发生发展。

在2007 - 2008年RSV感染高发季节,根据标准的医疗操作规程予以帕利珠单抗,对孕龄33至35周的早产儿(n = 52)进行一项6年的观察性前瞻性多中心病例对照研究。研究对象的父母通过使用报告卡和一种新型手机(基于互联网的报表系统)报告婴儿复发性喘息评估结果。主要终点是特应性哮喘的发病率。

444名早产儿入选,349人在1岁内接受帕利珠单抗治疗。然而在6岁时,治疗组(15.3%)和非治疗组(18.2%)之间特应性哮喘没有差异(p = 0.57)。而在治疗组和未治疗组中,医生诊断的复发性喘息分别为15.3%和31.6%(p = 0.003)。

给予早产儿帕利珠单抗预防不能抑制特应性哮喘的发生,但可明显降低6岁内复发性喘息的发生率。
