NEJM:大规模药物控制治疗疥疮,伊维菌素很有效

研究人员随机分配三个岛社区的人们接受三种不同的干预疥疮控制措施:标准氯菊酯治疗,涉及到受影响的人及与其接触的人(标准治疗组)、氯菊酯(氯菊酯组)、或伊维菌素组(伊维菌素组)。主要成果是从基线到12个月的疥疮和脓疱病的发病率和变化。

共有2051人参加报名;803名在标准护理组,伊维菌素组532名,苄氯菊酯组有716名。从基线到12个月,所有组疥疮的发生率都显著下降,伊维菌素组中下降的最多。在标准治疗组的患病率从36.6%下降至18.8%(相对减少发病率,49%;95%可信区间[Cl],37至60),氯菊酯组从41.7%到15.8%(相对减少,62%;95%Cl,49至75),伊维菌素组从32.1%到1.9%(相对减少,94%;95%Cl,83至100)。在所有组中脓疱疮的患病率也下降了,伊维菌素组中降低的最多。标准护理组的患病率从21.4%下降到14.6%(相对减少,32%;95%Cl,14至50),苄氯菊酯组中从24.6%下降至11.4%(相对减少,54%;95% Cl,35至73),伊维菌素组中从24.6%下降至8.0%(相对减少,67%;95%Cl,52至83)。不良反应轻微,伊维菌素组比菊酯组有更多的不良反应(15.6% vs 6.8%)。

大规模药物治疗,特别是伊维菌素的应用,能够有效控制疥疮和脓疱病。
