Radiology:肺MRI弹性配准在评估系统性硬化患者肺纤维化的价值

背景:目前尚无影像学方法能够敏感的评估肺纤维化所致肺功能变化。利用超短回波时间MRI能够用来对肺实质及肺运动进行评价。本研究旨在评价肺MRI吸气-呼气弹性配准来评估系统性硬化(SSc)患者肺纤维化的价值。

本研究共纳入了健康志愿者与SSc患者,并行胸部高分辨CT和胸部MRI。在3T MRI进行吸气-呼气后采用2次屏气,进行冠状位、3D、超短回波时间梯度回波序列肺部成像。利用弹性配准算法对吸气和呼气图像进行配准。在变形场中计算Jacobian行列式并在伪彩图中呈现。比较健康志愿者与SSc患者明显变形区域和Jacobian行列式算法<-0.15区域的相似性。利用ROC曲线分析验证诊断纤维化的最佳Dice相似性系数截断值。

结果为,共纳入了16名SSc患者和11例健康受试者。在健康志愿者下叶基底段和无非纤维化的SSc患者具有呼气末Jacobian行列式算法<-0.15显着缩减区域,而在纤维化患者中并未出现。以Dice 相似性系数0.36为最佳截断值,MRI评价高分辨CT纤维化患者纤维化的敏感性、特异性分别为86%、75% (ROC曲线下面积为0.81; P = .04)。

本研究表明,吸气-呼气MRI弹性配准显示系统性硬化相关肺纤维化呼吸形变较无纤维化患者少见。
