随着cTn检测的敏感性越来越高,观察到的2型心肌梗死(T2MI)的发生率有望随之增加。但是,如何诊断、如何进行风险分层,以及如何治疗T2MI患者仍有待确定。本研究旨在使用生物标记物来鉴别T2MI并进行风险分层。

纳入CHOPIN研究招募的因胸痛到急诊科就诊的患者进行回顾性分析。由两位心脏病专家独立的判定1型MI(T1MI)和T2MI。使用受试者工作特征曲线分析几种生物标志物(cTnI、肽素、CT-proET1、MR-proADM和降钙素原)单用和联合使用区分T2MI与T1MI的预后能力。于随访180天使评估上述生物标志物对全因死亡率和主要不良血管事件(急性心梗、不稳定心绞痛、再发心梗、心衰卒中)。

T1MI和T2MI患者六种生物标志物的表达水平

共2071位患者,其中诊断为T1MI和T2MI的患者分别有94例和176例。T1MI患者的起始cTnI水平更高,而T2MI患者的起始MR-proANP、CT-proET1、MR-proADM和降钙素原水平更高。

T1MI和T2MI患者在180天内的预后情况

根据CT-proET1、MR-proADM和MR-proANP诊断T2MI时的曲线下面积均高于cTnI的(0.765、0.750和0.733 vs 0.631)。结合所有的生物标志物,得出的准确性与使用临床变量和cTnI的模型的相似(0.854 vs 0.884, P=0.294)。

生物标志物诊断T2MI的敏感性和特异性

在临床模型中添加生物标志物获得在特征曲线下的最大面积(0.917)。在所有患者中,其他生物标志物(不包括cTnI)与180天的死亡率和主要的不良心血管事件相关,且在T1MI或T2MI的诊断之间没有相互作用。

生物标志物对180天预后的预测

评估反映T2MI发生的病理生理过程的生物标志物可能有助于将其与T1MI区别开来。不论心肌梗死的类型如何,本研究所检测的生物标志物(cTnI除外)都是患者预后的重要预测指标。
