Nature:如何混合样品才能最快速经济地检出新冠感染者?

最大限度地减少测试的数量,以降低成本,必须与最小化测试所需的时间,以减少感染的传播平衡。

抑制SARS-CoV-2很可能需要不断并快速地识别和隔离感染者。反转录聚合酶链反应(RT-PCR)检测准确,但费用昂贵,使得对每个人的定期检测花费颇高。这些费用对所有国家来说都是一个挑战,特别是对发展中国家。

我们已知的是,通过汇集(或合并)子样本并分组测试,可以降低费用。同时,我们必须在增加群体规模和保持检测灵敏度之间取得平衡,因为样本稀释会增加检测时采样区域内病毒载量低的个体出现假阴性的可能性。同样,最大限度地减少测试的数量,以降低成本,必须与最小化测试所需的时间,以减少感染的传播平衡。

最近,研究人员在Nature杂志发文,提出了一种基于超立方体的几何形状的子样本池的算法,在低流行率的情况下,在少量的测试和测试回合下,准确地识别感染者。

研究人员讨论了最佳的组大小,并解释了为什么考虑到疾病的高度传染性,主要将平行搜索作为首选。此外,研究人员还报告了概念验证实验,其中一个阳性子样本被阴性子样本稀释100倍后,仍然能够被检测到。研究人员量化了由于稀释而造成的灵敏度损失,并讨论了如何通过频繁的重新测试弥补这样的损失。

使用这些方法,大规模测试的成本可以降低一大截。目前,该方法正在卢旺达和南非进行实地试验。

大规模使用群体测试来密切和持续监测人口中的感染情况,同时快速有效地隔离感染者,为长期控制COVID-19提供了一条有希望的途径。
