OCC 2022:赵水平教授:"理想"胆固醇水平的人为何仍患动脉粥样硬化?

上世纪七、八十年代,我国人群胆固醇水平并不是很高,仍然可以看出胆固醇水平与冠心病死亡的关系。从1984~1999年,北京人群总胆固醇水平升高24%,冠心病死亡率也成倍增加,而胆固醇水平的升高对冠心病死亡的增加贡献了77%。

胆固醇与动脉粥样硬化之间关系的明确,经历了从病理、动物实验、流行病学到大规模临床干预试验的漫长科学历程,最终形成了公认的胆固醇理论,胆固醇是动脉粥样硬化发生的根本原因防治动脉粥样硬化已经成为我国21世纪提高居民健康水平的重中之重。

在第十六届东方心脏病学(OCC 2022)会议上,中南大学湘雅二医院心血管内科赵水平教授以“理想胆固醇水平人群为何仍有动脉粥样硬化的发生?”为题,对相关内容进行了详细的讲解。

赵水平教授认为,胆固醇会直接引起动脉粥样硬化性心血管病。原因有二点,第一,胆固醇(以脂蛋白形式)进入血管壁内是动脉粥样硬化发生的必备条件;第二,含有apoB脂蛋白(特别是LDL)是动脉粥样硬化性心血管病致病元凶或称病因,其他危险因素都只是”帮凶”。

低密度脂蛋白胆固醇(LDL-C)在动脉粥样硬化启动启、进展、并发症等阶段均起重要作用。1913年俄国病理学家阿尼茨科夫(Anitschkow)使用富含胆固醇的葵花油喂养兔,在短短的70天内就发生了动脉粥样硬化,从而成功了建立了世界上首个动脉粥样硬化动物模型,开创了动脉粥样硬化机制探索的里程碑。遗憾的是,阿尼茨科夫及后继者尝试在其他动物(鼠和狗)身上,用同样的高胆固醇饮食喂养方法,却始终未能成功制造出动脉粥样硬化模型。因此,阿尼茨科夫提出的“胆固醇致动脉粥样硬化”学说没有受到当时医学界的足够重视和普遍认可。后来人们发现,鼠(以及狗)实验失败的原因,是因为自然状态下的鼠和狗具有能够高效代谢胆固醇的代谢系统,不会因为饮食中的胆固醇过高发生动脉粥样硬化。

动脉粥样硬化又是一种“年轻”的疾病。家族性高胆固醇血症(FH)患者由于存在先天的胆固醇代谢基因缺陷,导致其低密度脂蛋白(LDL)受体数量减少或功能异常。这类患者体内胆固醇含量尤其是LDL-胆固醇(LDL-C)含量远高于正常人,使得患者皮肤上存在大量黄色瘤。他们往往在青少年甚至儿童时就因冠心病心肌梗死而夭折。

关于胆固醇在人类动脉粥样硬化中的作用的机制研究,在20世纪40年代才真正开始,因为之前人们普遍认为斑块是衰老的必然后果,无法预防

动脉粥样硬化斑块的形成是一个缓慢的过程。最近的一种假设表明,在心血管疾病危险因素(如吸烟、高血脂)的长期暴露下,血液循环中的单核细胞(一种白细胞)粘附在血管床内皮上,产生炎症反应使血管内皮损伤。单核细胞进入血管内皮下分化为·巨噬细胞。此时富含胆固醇的低密度脂蛋白(LDL)颗粒进入动脉血管内皮下并被氧化,随后被巨噬细胞吞噬后形成泡沫细胞,逐渐发展为粥样斑块。

因此,富含胆固醇脂蛋白(主要是LDL)穿内皮细胞转运是动脉粥样硬化的初始步骤。也体现在:

  •   LDL是血浆中最丰富的致动脉粥样硬化脂蛋白,
  •  LDL也是胆固醇向内壁内转运的关键载体,
  •  LDL穿内皮细胞转运不但是动脉粥样硬化发生的初始步骤,也是LDL发挥致动脉粥样硬化作用的必有过程。

动脉粥样硬化的形成过程在一定程度上是可以逆转的。但是随着年龄的增长以及动脉粥样硬化不断发展,这种可逆转趋势越来越弱。全生命过程中保持较低的血脂水平,尽量减少低密度脂蛋白和其他apoB脂蛋白累积暴露,有可能显著改变动脉粥样硬化斑块的进程,并大大降低心血管事件的终生风险。

对于50-70岁的人群来说,降低1.0mmol/L(40mg/dL)LDL-C,可将心肌梗死风险降低25%,如果能够在整个生命过程中均保持低水平的LDL-C,将比晚年去降脂健康获益更大。

否则,脂蛋白就像小米粒一样,不断地沉积至血管壁内“煮粥”,粥样斑块形成并逐渐增大。

尽管大多数人一生中均可能发生动脉粥样硬化,但并非所有人都将不可避免地发生冠心病或者缺血性心血管事件。换而言之,在人群中广泛发生动脉粥样硬化这一背景下,仅有部分人群会遭受冠心病困扰。最后,赵水平教授总结道,没有“理想”胆固醇水平的人群,但每一个体有其“理想”胆固醇水平。对于个体“理想”的胆固醇水平来说,它有个标准,具体是含胆固醇的脂蛋白不进入动脉壁内,或含胆固醇的脂蛋白不在动脉壁内滞留。
